CT/MRI影像融合在鼻咽癌适行放疗靶区勾画中的应用 被引量:6

摘要 目的探讨CT与CT/MRI图像融合在鼻咽癌适形调强放射治疗中肿瘤靶区勾画的精确性和一致性。方法选择35例经鼻咽镜活检病理和鼻咽部CT/MRI证实为Ⅰ~Ⅳa期的初治鼻咽低分化鳞癌患者35例,分别对患者的CT图像和CT/MRI融合图像进行靶区勾画,并制定IMRT治疗计划。然后比较两个计划中95%靶区容积(D95)所受的剂量,以及各期鼻咽癌CT与CT/MRI融合图像的靶区相容情况。结果用CT图像上勾画的GTV做IMRT治疗计划时,GTV得到的平均D95剂量是67.78Gy,而CT/MRI上的GTVf的平均D95剂量是60.03Gy,这说明GTVf内有一部分肿瘤处于低剂量区。而用CT/MRI融合图像上勾画的GTVf做IMRT治疗计划时,二者的剂量分布基本相同。结论 CT图像上勾画的GTV包含CT/MRI融合图像上勾画的GTVf主要集中在T1和T2病变,而T3、T4病变主要是GTVf包含GTV。而且在不同的分期,GTV与GTVf之间均具有互补关系。
