某社区卫生服务中心糖尿病患者规范管理后的效果分析

Analysis of A Community Health Service Centers Standardize Management of Patients with Diabetes after Effects

目的评价社区卫生服务中心糖尿患者规范管理后治疗效果。方法将2005年至2008年长期在张掖市人民医院东街社区卫生服务中心就诊治疗的270例2型糖尿病患者建立慢性病管理档案,进行规范管理,制定个案治疗方案,进行个体化生活方式指导,定期检测血糖,对规范管理前后综合指标进行分析。结果①规范管理前后比较,空腹血糖[(7.2±2.0)vs(8.1±4.8)mmol/L,P<0.05]、餐后2h血糖[(9.0±2.5)vs(10.9±7.7)mmol/L,P<0.01]、糖化血红蛋白[(7.2±1.7)%vs(8.4±2.3)%,P<0.01]均显著下降;②糖尿病知识规范管理后较管理前明显上升(P<0.01);③管理后愿意接受综合治疗率由46.7%提高至77.4%(P<0.01),整体血糖控制从29.3%上升至64.8%(P<0.01)。结论糖尿病患者规范管理后血糖控制达到最佳水平。

Objective To evaluate the effect of the standardized management of diabetic patients in the community of health service center. Method To set 270 cases of type 2 diabetes mellitus （T2DM） were set up chronic disease file, checked by the doctor long-time in the east street community of people hospital of Zhangye from 2005 to 2008 years, and standardized management. Develop the individualized treatment programs and guide lifestyle. Regularly monitor blood glucose. Analyze the data before and after standardized management. Result （1）Compare the data of fasting blood glucose [（7.2±2.0）vs（8.1±4.8）mmol/L, P〈0.05], blood glucose after meal for 2 hours [（9.0±2.5）vs（10.9±7.7）mmol/L, P〈0.01], and glycated haemoglobin [（7.2±1.7）vs（8.4±2.3）%, P〈0.01], were significantly lower than before. （2）The knowledge of DM were increased （P〈0.01）. （3）Patients in the management accepting this program increased from 46.7% to 77.4% （P〈0.01）, the rate of blood glucose level controlled significantly increased from 29.3% to 64.8% （P〈0.01）. Conclusions Standardized management can succeed in controlling the patients＇ blood glucose.
