摘要 目的检测IL-23/IL-17轴在鼻息肉中的表达水平,探讨IL-23/IL-17轴在鼻息肉发病中的作用。方法收集在鼻内镜下慢性鼻-鼻窦炎鼻息肉手术患者鼻息肉组织(鼻息肉组)45例,因鼻中隔偏曲行下鼻甲部分切除术患者(对照组)下鼻甲黏膜组织30例,分别采用免疫组织化学方法和RT-PCR方法检测两组IL-23和IL-17的表达情况,并对两组IL-23和IL-17表达水平之间的关系进行研究。结果 RT-PCR显示,鼻息肉组IL-23p19mRNA和IL-17mRNA表达水平明显高于对照组[(0.676±0.077)vs(0.212±0.015);(0.681±0.044)vs(0.220±0.044)],两组比较差异均具有统计学意义(P<0.01);免疫组化显示,鼻息肉组可见大量IL-23和IL-17阳性炎性浸润细胞,单位目标面积光密度均值明显高于对照组[(0.321±0.023)vs(0.199±0.011);(0.315±0.023)vs(0.211±0.029)],两组比较差异均有统计学意义(均为P<0.01);IL-17表达水平与IL-23表达水平呈正相关关系(基因表达水平r=0.723,P<0.01;蛋白表达水平r=0.991,P<0.01)。结论 IL-23、IL-17可能参与了慢性鼻-鼻窦炎鼻息肉的发病过程,并且IL-23可能通过调解鼻腔局部IL-17水平而发挥作用。 Objective To investigate the expression of interleukin - 23 (IL - 23 ) and interleukin - 17 ( IL - 17 ) axis in the nasal polyp patients and explore the role of IL - 23/IL - 17 axis in the nasal polyp. Method Forty - five nasal polyp(NP) patients' s NP tissue and 30 simple deviation of nasal septum patients' normal inferior turbinate tissue were collected, mRNA and protein expres- sion of IL -23 and IL - 17 were detected by immunohistoehemistry and RT -PCR. SPSS 17.0 software package was used for statistical analysis, test and Spearman coefficient were used to analyse and compare the expression level and the correlation. Results RT - PCR demomstrated that the average expression level of IL - 23p19mRNA and IL - 17mRNA in NP tissues [ (0. 676± 0. 077 ) , (0. 681± 0.044) ] had significant differences (P 〈 0.01 ) from those in normal inferior turbinate tissues [ (0. 212 ± 0. 015 ), (0. 220 ±0. 044)]. Immunohistochemical staining demomstrated that there were much more IL -23 and IL - 17 positive cells in NP patients than in the normal controls. The average expression levels of IL- 23 and IL- 17 protein in NP tissues [ (0. 321± 0. 023 ), (0.315 ± 0.023) ] were significantly different (P 〈 0.01 ) from those in normal inferior turbinate tissues [ (0. 199 ± 0. 011 ), (0. 211 ± 0. 029) ]. IL - 17mRNA positively correlated with IL - 23pl 9mRNA ( r = 0. 723, P 〈 0.01 ), and IL - 17 protein positively correlated with IL - 23 protein ( r = 0.991, P 〈 0. 01 ). Conclusion IL - 23 and IL - 17 may involved in the chronic inflammation of NP. IL - 23 may be play a role through adjusting the level of IL - 17.
