老年消化性溃疡96例临床分析

Clinical Analysis of 96 Elderly Patients with Peptic Ulcer

目的总结、探讨老年消化性溃疡的临床特点,为临床诊断提供帮助。方法回顾分析96例老年消化性溃疡患者的临床表现、胃镜资料及并发症情况,与同时期检出的120例中青年消化性溃疡资料进行对照。结果老年消化性溃疡患者缺乏典型节律性腹痛症状,与中青年组存在统计学差异(P<0.01);溃疡部位以胃溃疡为主,且高位溃疡显著增多,与中青组存在差异(P<0.05);合并并发症者较中青年组显著增加(P<0.01),其中合并出血和癌变的发生率明显高于中青年组,具有统计学差异(P<0.01)。结论老年消化性溃疡的临床表现多不典型,且伴发疾病多,并发症发生率高而且严重。因此对疑似老年消化性溃疡的患者应尽早进行胃镜等客观检查,力争早诊断、早治疗。

Objective To summarize and explore the clinical characteristics of elderly peptic ulcer,to provide help for the clinical diagnosis. Methods Retrospective analysis of 96 cases of elderly patients with peptic ulcer clinical manifestations,endoscopy data and complications,with the same period of the 120 cases detected in young peptic ulcer data for comparison. Results Elderly peptic ulcer group lack the typical symptoms of abdominal pain,and a non-elderly group with a significant difference （P0.01）;gastric ulcer and high position ulcer increased significantly than the the non-elderly group （P0.05）;complications compared with the non-elderly group was significantly increased （P 0.01）,which bleeding and cancer incidence was significantly higher than that of the non-elderly group（P 0.01）.Conclusion Elderly peptic ulcer had untypical clinical symptoms and concomitant diseases,high incidence of complications. Gastroscopy examination thus of suspected elderly peptic ulcer patients should be carried out as soon as possible,and strive to early diagnosis,early treatment.
