中药治疗连续60个月的565例HIV/AIDS病人的CD_4T淋巴细胞消长特征

A sixty-month study of the change of CD_4T-lymphocyte cell count treated with traditional Chinese medicine in 565 HIV/AIDS patients

目的观察连续接受中医药治疗60个月的艾滋病病毒(HIV)感染者及艾滋病(AIDS)病人(HIV/AIDS病人),CD4T淋巴细胞计数的变化。方法收集全国19个省(自治区、直辖市)从2004-2011年采用中医药治疗的HIV/AIDS病人565例,对6个时间点(治疗0、12、24、36、48、60个月)的CD4T淋巴细胞计数变化、分层分析(≤200个/mm3、201～350个/mm3、351～500个/mm3、>500个/mm3)比较。结果 CD4T淋巴细胞计数≤200个/mm3以西药抗病毒治疗为主,中医药为辅,中西医组占90.90%(100/110),CD4T淋巴细胞计数平均每年上升约6个/mm3;中医组占9.09%(10/110),CD4T淋巴细胞平均每年下降约7个/mm3。CD4T淋巴细胞计数201～350个/mm3以中医药治疗为主,中医组占80.73%(155/192),CD4T淋巴细胞平均每年下降约3个/mm3;中西医组占19.27%(371/192),CD4T淋巴细胞平均每年下降约2个/mm3。CD4T淋巴细胞计数351～500个/mm3以中医药治疗为主,中医组占92.19%(118/128),CD4T淋巴细胞平均每年上升约3个/mm3;中西医组占15.63%(20/128),平均每年上升约8个/mm3。CD4T淋巴细胞计数>500个/mm3以中医药治疗为主,中医组占92.00%(115/125),CD4T淋巴细胞平均每年上升约4个/mm3;中西医组占8.00%(10/125),CD4T淋巴细胞平均每年上升约14个/mm3。结论长期服用中医药可使CD4T淋巴细胞保持稳定或者延缓其下降速度,远期疗效较好。

Objective To evaluate the therapeutic effect through observing the changes of CD4 T-lymphocyte cell （CID4） count in 565 cases with HIV/AIDS treating continuously with traditional Chinese medicine（TCM） for 60 months. Methods Information of 565 HIV/AIDS patients coming from 19 provinces and cities and treated with TCM in the year 2004--2011 was collected. Comparisons were performed to the changes of the indexes of CD4 counts （≤ 200copies/,m3 , 201 -- 350 copies/mm3 , 351- 500copies/mm3 and 〉500copies/mm3 ） at 6 time points （before treating, 12, 24, 36, 48 and 60 months）. Results In CD4 count≤ 200 copies/mm3 , the treatment was mainly with western medicine therapy and sup- plemented by TCM, CD4 increased by an average annual rate of about 6 copies/mma in 100 cases （90. 900/6o） of TCM plus western medicine group, but CD4 declined by an average annual rate of about 7 copies/mm3 in 10 cases （9. 09% ） of TCM group. For CD4 count 201--350 copies/mm3 , the treatment was mainly with TCM, CD4 declined by an average annual rate of about 3 copies/mms in 155 cases （80. 73%） of TCM group, CD4 declined by an average annual rate of about 2 copies/ mma in 37 cases （19. 27%） of TCM plus western medicine group. For CD4 count 351--500 copies/mm3, the treatment was mainly with TCM, CD4 increased by an average annual rate of about 3 copies/mma in 118 cases （92.19%） of TCM group, CD4 increased by an average annual rate of about 8 copies/mm3 in 20 cases （15.63%） of TCM plus western medi- cine group. For CD4 count〉500 copies/mm3 , the treatment was mainly with TCM, CD4 increased by an average annualrate of about 4 eopies/mm3 in 115 cases （92.00%） of TCM group, CD4 increased by an average annual rate of about 14 copies/mm3 in 10 cases （8. 00%） of TCM plus western medicine group. Conclusion Long-term use of TCM could maintain or slow the pace of CD4 count deeli-ning in patients with HIV/AIDS and may achieve lasting effectiveness.
