Surgical treatments of pancreatic cancer have been improved greatly in the recent century. For a long time, vascular invasion of tumor was contraindicated for surgery. With the development of surgical skills and principles, patients with tumor invasion of the superior the mesenteric vein / the portal vein could be treated by combined resection and reconstruction of the superior mesenteric vein / the portal vein by pancreaticoduodenectomy, which greatly improved the resection rate and the 5-year survival rate. A reasonable surgical option could significantly improve the clinical outcome of the patients.
