十二指肠空肠吻合治疗十二指肠严重损伤

Duodenojejunostomy in the treatment of severe duodenal injury

探讨十二指肠损伤的处理方法及预后。方法回顾性分析10例患者十二指肠严重损伤的外科处理方式。经以往各种手术相比较,十二指肠破口与空肠直接Roux-en-Y吻合效果最好。结果本组10例十二指肠严重损伤患者,经十二指肠破口与空肠直接行Roux-en-Y吻合,术后恢复顺利,痊愈出院,无围手术期死亡。除1例患者3.5年后肿瘤复发死亡外,其余患者无并发症及后遗症。结论十二指肠破口与空肠直接行Roux-en-Y吻合,手术方法简单,创伤轻,疗效好,而且适用于十二指肠各部位的损伤。

Objective To discuss the treatment of patients with duodenal injury. Methods A retrospective analysis was made of 10 patients with severe duodenal injury who had undergone surgical treatment. Roux-en-Y combined with duodenojejunostomy is better than the previously used method. Results Ten patients with severe duodenal injury were cured successfully by Roux-en-Y and they were discharged uneventally. One patient died from the recurrence of cancer 3.5 years after operation, and the rest of the patients had no complications and sequelae. Conclusion Roux-en-Y procedure has a simple, less traumatic, and better curative effect. It is appropriate for the treatment of different duodenal injury.
