宫颈癌术前介入辅助化疗18例

Interventional Treatment with Adjuvant Chemotherapy Before Operation of Cervical Carcinoma

探讨表阿霉素、顺铂、环磷酰胺在宫颈癌手术前介入化疗的方法及疗效观察。方法 :对 18例 (2 2例次 )宫颈癌 (Ⅱa期 4例 ,Ⅱb期 14例 )术前行介入化疗 ,14例行单侧动脉栓塞 ,4例行双侧动脉栓塞 ,化疗 14d后行宫颈癌根治术 ,进行疗效、毒副作用的观察和评定。结果 :术前介入治疗症状减轻率 10 0 % ,近期有效率 10 0 %。术后见各断端均无癌变 ,肿块范围缩小 ,淋巴结转移 1例 ,低于估计值。随访 14例均健在 ,2例局部复发行放疗 ,生存期 3～ 32个月。结论 :宫颈癌术前用顺铂、表阿霉素、环磷酰胺介入加栓塞辅助化疗近期疗效好、毒副作用低且增加手术的机会 ,是宫颈癌辅助化疗的理想药物和方法。

Objective:To study the method of cervical carcinoma treated with eprirubicin (EPI), cisplatin (DDP), and methotrexate (MTX) by intervening adjuvant chemotherapy before operation. Methods:Before radical hysterectomy, 18 cases (22 person-time) of cervical carcinoma were treated with interventional therapy. Of 18 cases, 4 cases were in stage Ⅱa, 14 in stage Ⅱb. Among them, the one-sided uterine arteries of 14 cases and two sided uterine arteries of 4 cases were embolized, radical hysterectomy for cervical cancer were performed after 14 days. Then the curative effect, toxic, and side effect were observed and evaluated. Results:The remission rate of interventional treatment before operation was 100%and the effective rate at the near future (tumor extinction rate) was 100%. While operating, there were not any cancer cells in the remained ends and the tumor became smaller, a lymph node with cancer cells was found in one case. 14 cases follwed up weresurvival:14 patients survived for 3-32 months, 2 relapsed, 4 lost the relation with us. Conclusion:Interventional treatment with EPI, DDP, and MTX is effective with less side effects, making the patients obtain the more treatable chances of radical hysterectomy for cervical cancer, it is a ideal method which the drugs are combined with adjuvant chemotherapy for cervical carcinoma.
