认知行为疗法联合艾司西酞普兰治疗产后抑郁障碍疗效观察

Efficacy of cognitive behavior therapy combined with escitalopram in patients patients with post - partum depression

目的:探讨认知行为疗法联合艾司西酞普兰治疗产后抑郁障碍的疗效。方法:58例产后抑郁障碍患者随机分为认知行为疗法联合艾司西酞普兰组(n=30)和单用艾司西酞普兰组(n=28)。疗程6周。采用爱丁堡产后抑郁量表(EPDS)和汉密尔顿抑郁量表(HAMD)评定临床疗效;采用不良反应量表(TESS)评定药物不良反应。结果:疗后两组HAMD和EPDS评分均下降,CBT组下降更明显,疗后第6周末,两组间HAMD和EPDS评分比较有明显差异(P<0.05)。CBT组的药物剂量低于对照组(P<0.05)。两组未见明显不良反应。结论:认知行为疗法联合艾司西酞普兰治疗产后抑郁障碍的疗效肯定,能改善患者产后适应生活的能力和人际关系。

Objective： To explore clinical effects of cognitive behavior therapy combined with escitalopram in patients with post-partum depression.Methods： 58 patients with post-partum depression disorder were randomly divided into cognitive behavior therapy（CBT） combined with escitalopram group and single escitalopram group.The course of treatment was 6 weeks.The efficacy was evaluated with Hamilton Depression Rating Scale（HAMD） and Edinburgh Postnatal Depression Scale（EPDS） before and 2,4,and 6 weeks after the treatment.The side effects were assessed by treatment emergent symptom scale（TESS）.Results： After the treatment,the scores of HAMD and EPDS decreased in both groups,and those of CBT group decreased significantly.Moreover,there were signifi-cant differences in the HAMD and EPDS score between the two groups 6 weeks after the treatment（P ＆lt; 0.05）.The dose of escitalopram in CBT group was more lower than that in control group（P ＆lt; 0.05）.There were no obvious side effects in the two groups.Conclusions： The cognitive behavior therapy combined with escitalopram could relieve depression symptom and improve life adaptability and interpersonal relationships in patients with post-partum depression.
