慢性心力衰竭患者血清中生长分化因子-15水平测定意义研究

摘要 目的通过检测生长分化因子-15(growth differentiation factor,GDF-15)水平来探讨其与慢性心力衰竭(chronic heart failure,CHF)的关系,并与脑钠肽(brain natriuretic peptide,BNP)比较其在CHF诊断中的价值。方法 2010年12月—2013年4月共观察CHF患者100例,作为CHF组,正常对照组40例,来自查体中心健康查体者。患者入院第2天清晨采集空腹静脉血3~4 ml,GDF-15含量采用ELISA法检测,采用床旁Triage BNP测定仪测定同期血清BNP的含量。肝、肾功能结果使用日立7600全自动生化分析仪检测。超声心动图检查:入院后2 d做超声心动图,使用为美国ACUSON公司生产XP128彩色超声诊断仪(探头频率2.5~4.0 MHz),受试者仰卧位,按标准方法取左心长轴切面及心尖五腔切面,计算左心室射血分数(ejection fraction,EF)。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果 CHF组GDF-15(2 565.3±798.8)ng/L,对照组(801.6±218.4)ng/L,两组比较差异有统计学意义(P<0.05)。CHF组B、C、D期患者GDF-15浓度与BNP浓度明显高于对照组(P<0.05),左心室EF值明显低于对照组(P<0.05),并随着心功能分级的增高呈升高趋势(P<0.05)。CHF组血清GDF-15浓度与血清BNP浓度呈显著线性正相关(r=0.735,P<0.05);与EF值呈显著线性负相关(r=-0.671,P<0.05)。结论检测GDF-15浓度是诊断CHF重要的实验室指标,可作为心力衰竭早期诊断的生物标志物。
