摘要
【目的】探讨2型糖尿病肾病不同阶段以及糖尿病不同合并症时肾功能临床指标的相互关系。【方法】收集2型糖尿病并应用胰岛素强化治疗的患者171例,分别按GFR、SCr不同程度分组,进一步将患者分为单纯糖尿病、糖尿病合并高血压病、糖尿病合并高血压和高脂血症三组再进行相关分析。【结果】整体来看,SCr、UA是2型糖尿病患者GFR下降的独立相关因素;U-β2-MG与A1β、IgG、α1-MG、SBP、SCr显著相关;主成分分析从18个临床指标中提取出10个互不相关因子的累计贡献率超过85%,其中主成分C1的贡献率最高。【结论】糖尿病患者的肾功能改变与多种因素有关,GFR是评估糖尿病肾病时肾功状态的良好指标,肾小管功能改变是糖尿病肾病的重要组成部分;从变异程度、相关性、对主成分的因子负荷等角度评价,Scr是肾功能评估较为优越的评价指标。
[Objective] Type 2 diabetic nephropathy at different stages and different diabetes complications in renal function relationships of clinical indicators. [MethOd] Collected in patients with type 2 diabetes and insulin intensive therapy in 171 cases, respectively, according to the GFR, degree of SCr different grouping, further divided the patients into pure diabetes, hypertension, diabetes, hypertension and diabetic hyperlipemia three groups and then to correlation analysis. [Result] Overall, SCr, UA is the independence of GFR decline in patients with type 2 diabetes related factors; U - beta - 2 MG 1 - MG with A1, alpha, beta, IgG, SBP, SCr significant correlation; Principal component analysis (18 clinical indicators from 10 unrelated factors the cumulative contribution rate of more than 85%, including principal component C1 has the highest contribution rate. [Conclusion] Diabetic patients renal function change is related to many factors, GFR is evaluating diabetes kidney disease - a good indicator of renal function state, renal tubular function change is an important component of the diabetic nephropathy; From the degree of variation, correlation and factor loading of principal component evaluation, Scr is renal function evaluation more superior evaluation index.
