艾滋病住院患者的住院费用及其影响因素研究

Hospitalization cost of AIDS patients and its influencing factors

目的通过分析影响艾滋病住院患者住院费用的主要因素,为满足我国艾滋病患者的医疗服务需求,节约医疗成本提供一定的科学依据。方法对2008年1月～2010年12月河南省某三级传染病专科医院所有进行过艾滋病抗病毒治疗的1 423名艾滋病住院患者进行问卷调查,对住院总费用的影响因素进行单因素分析和多元回归分析。结果单因素分析显示,不同性别、CD4基线水平和职业的艾滋病患者的住院总费用之间差异有统计学意义(P<0.01或P<0.05),不同年龄和住院次数间差异无统计学意义(P>0.05);随着住院天数的延长,患者的医疗费用递增(P<0.01)。多元线性回归分析显示,住院天数、CD4基线水平是住院总费用的影响因素。结论艾滋病住院患者的医疗费用与住院天数及CD4基线水平有关。

Objective To analyze the hospital expense of patients with AIDS and the major factors influencing the cost,in order to provide scientific basis for meeting the demand for medical services and for reducing the cost as well.Methods A total of 1 423 patients with AIDS hospitalized in a tertiary infectious hospital for antiviral therapy from January 2008 to December 2010 in Henan province were recruited and all completed a questionnaire survey.Single factor and multiple regression analysis were used to determine the influencing factors of the total hospitalization cost.Results Univariate analysis showed that gender,CD4 baseline level and occupation could significantly affect the total hospitalization cost of AIDS patients（P〈0.0 or P〈0.05）,while age and number of times of hospitalization could not（P〈0.05）.The hospital expense increased gradually with the extension of hospital stay（P〈0.0）.Multiple regression analysis also showed that the total hospitalization cost of patients with AIDS was closely related to the length of hospital stay and the CD4 baseline level.Conclusions The hospitalization days and CD4 baseline level are the influencing factors of the total hospitalization cost of patients with AIDS.Therefore,it is proposed that patients with no or mild AIDS/HIV symptoms should receive community-based home nursing.
