产后抑郁症患者的述情障碍及心理护理干预

Alexithymia of Patients with Postpartum Depression and Psychological Nursing Intervention

目的探讨产后抑郁症患者的述情障碍及心理护理干预对产后抑郁症疗效的影响。方法将60名产后抑郁症患者随机分为心理护理干预组、非心理护理干预组,治疗6周,分别给予心理护理干预配合药物治疗和单独使用药物治疗,采用多伦多述情障碍量表20项(TAS-20)、汉密尔顿抑郁量表17项版本(HAMD-17)、临床疗效总评量表病情严重程度(CGI-SI)进行评定。所有数据输入SPSS17.5软件包进行统计分析。结果①治疗前两组间TAS-20总分及各因子分比较差异无统计学意义,治疗后第6周心理护理干预组TAS-20总分及各因子分显著低于非心理护理干预组,差异有统计学意义(t=-12.286,-3.501,-9.487,-13.152;P<0.01)。治疗后第6周,心理护理干预组TAS-20总分及各因子评分显著低于治疗前,差异有统计学意义(t=27.867,8.209,16.351,29.000;P<0.01),非心理护理干预组TAS-20总分及各因子评分与治疗前相比较,差异无统计学意义;②治疗前两组间HAMD评分比较差异无统计学意义,治疗后第6周心理护理干预组HAMD评分显著低于非心理护理干预组,差异有统计学意义(t=-2.548,P<0.05)。治疗后第6周,两组HAMD评分均显著低于治疗前,差异有统计学意义(t=28.387,14.300;P<0.01);③治疗前两组间CGI-SI评分比较差异无统计学意义,治疗后第6周心理护理干预组CGI-SI评分显著低于非心理护理干预组,差异有统计学意义(t=-2.408,P<0.05)。治疗后第6周,两组CGI-SI评分均显著低于治疗前,差异有统计学意义(t=13.293,11.696;P<0.01)。结论产后抑郁症患者存在明显的述情障碍,心理护理干预能明显改善产后抑郁症患者的述情障碍和抑郁症状,提高其生活质量。

Objective To detect the alexthymia of patients with postpartum depression and the effect of psychological nursing intervention on postpartum depression. Methods A total of 60 patients with postpartum depression were divided into psychological nursing intervention group and non-psychological nursing intervention group for 6 weeks, they were given psychological nursing in- tervention with drug therapy or single drug treatment,Chinese version of the 20--item Toronto Alexithymia Scale （TAS--20）, HAMD and CGI--SI were used to evaluate the efficacy before and after the treatment. Results ①To compare the baseline TAS-- 20 scores and Tfl ,Tf2,Tf3 factors scores,there were not statistically significant between psychological nursing intervention group and non-psychological nursing intervention group（P〉0. 05） ,At the end of the 6nd week, TAS--20 scores and Tfl,Tf2 ,Tf3 fac- tors scores of patients in psychological nursing intervention group were reduced significantly than those in non-psychological nursing intervention group（t=-12： 286,-3. 501,-9. 487,-13. 152;P〈0. 01）. At the end of the 6nd week,TAS--20 scores and Tfl ,Tf2, Tf3 factors scores of patients in psychological nursing intervention group were reduced significantly than the baseline（t= 27. 867, 8. 209,16. 351,29. 000 ;P〈0.01）, and there were not statistically significant in non-psychological nursing intervention group（P〉 0.05）. ②To compare the baseline, in the total score of HAMD of patlents,there were not statistically significant between psycho- logical nursing intervention group and non-psychological nursing intervention group （P〉0. 05）iAt the end of the 6nd week, the total score of HAMD of patients in psychological nursing intervention group was reduced significantly than that in non-psychologi- cal nursing intervention group （t=2. 548,P〈0.05）. At the end of the 6nd week, the total score of HAMD of patients in psycho- logical nursing intervention group and non-psychological nursing intervention group were reduced significantly than the baseline （t=28. 387,14. 300;P〈0.01）. ③To compare the baseline total score of CGI--SI,there was not statistically significant between psy- chological nursing intervention group and non-psychological nursing intervention group （P〉0. 05） ,At the end of the 6nd week, the total score of CGI--SI of patients in psychological nursing intervention group was reduced significantly than that in non-psycho- logical nursing intervention group （t=-2. 408,P〈0.05）. At the end of the 6nd week, the total score of CGI--SI of patients in psychological nursing intervention group and non- psychological nursing intervention group were reduced significantly than the baseline （t=13. 293,11. 696;P〈0.01）. Conclusion Patients with postpartum depression have significant alexithymia;Psychologi- cal nursing intervention can obviously relieve alexithymia and depressive symptoms of patients with postpartum depression and im- prove their life quality.
