Investigation of Drug Repercussion in Outpatient Pharmacy during 2009 - 2012

目的分析门急诊退药的主要原因,探讨退药的处理方法和改进措施。方法以门急诊2009～2012年1 095张退药处方为研究对象,对退药的原因和退药的品种进行分类、统计,应用帕累托图分析退药的主要原因。结果药品不良反应、不合理用药和检查项目取消为退药的主要因素;患者拒绝用药、输液困难、药价因素为退药的次要因素;收费问题、药品质量等为一般因素。结论根据退药的主要原因,健全医院的退药制度,提高临床医师的业务素质;发挥好药师的作用。

Objective To investigate the reasons for drug repercussion in outpatient pharmacy and explore the preventive measures. Methods A total of 1 095 prescriptions of drug repercussion in outpatient pharmacy of the hospital were collected during 2009 -2012. The causes for drug repercussion and the kinds of the returned drugs were analyzed by Pareto Chart. Results Adverse drug reaction, irrational drug application, and cancel of the test project were the most common reasons for the drug repercussion; refusing of drug administration, transfusion difficulty and price of the drugs were the second common reasons; drug expenses and drug quality were other reasons. Conclusion The drug repercussion system need to be refined, the professional quality of the doctors need to be improved, and the role of the pharmacists should be fully played to reduce the drug repercussion.
