气化逐瘀法联合抗生素治疗ⅢA型前列腺炎的临床研究

Bladder gasification and stasis dispersion combined with antibiotic therapy for ⅢA chronic prostatitis

目的:ⅢA型前列腺炎的病机特点为"湿热、膀胱气化不利、瘀浊阻滞",本研究评价气化逐瘀法联合抗生素治疗ⅢA型前列腺炎的临床疗效。方法:采用随机对照临床试验研究方法,将符合ⅢA型前列腺炎诊断的患者随机分为A、B、C 3组,3组均给予口服司帕沙星,B组同时口服坦索罗辛,C组给予司帕沙星+中药汤剂。疗程均为4周。主要疗效指标为中医证候总评分和慢性前列腺炎NIH-CPSI总评分,次要指标为前列腺液(EPS)中白细胞计数。结果:治疗后中医证候总评分B组(42.15±10.29)分和C组(41.26±11.25)分、NIH-CPSI总评分[B组(13.25±6.04)分、C组(12.38±7.19)分]较A组[(49.43±11.09)分、(17.62±5.84)分]明显降低(P均<0.05),且C组中医证候总评分治疗前后的差值(12.65±11.76)较B组(8.55±10.15)更显著(P<0.05);C组治疗后EPS白细胞计数较A组和B组明显降低(P<0.05)。结论:气化逐瘀法联合抗生素治疗ⅢA型前列腺炎有较好的疗效,值得临床推广运用。

Objective： To evaluate the clinical efficacy of bladder gasification and stasis dispersion combined with antibiotic therapy in the treatment of IIIA chronic prostatitis. Methods： We conducted a randomized controlled clinical study on 120 IliA prostatitis patients that all met the diagnostic criteria. We divided the patients into groups A, B and C of equal number to receive oral medication of spadloxacin, sparfloxacin 4- tamsulosin, and sparfloxacin ＋ herbal decoction, respectively, all for a course of 4 weeks. We evaluated the primary therapeutic indexes according to the total scores of the patients on traditional Chinese medicine （TCM） syndrome and NIH-CPSI and the secondary therapeutic indexes based on the count of white blood cells （WBC） in the expressed prostatic secretion （EPS）. Results： After treatment, the total scores on TCM syndrome and NIH-CPSI were significantly reduced in groups B （42.15 ± 10.29 and 13.25 ±6.04） and C （41.26 ± 11.25 and 12.38 ±7.19） than in A （49.43 ± 11.09 and 17.62 ±5.84） （ P 〈 0.05）, and so was the WBC count in EPS in group C （7.76 ± 15.73） than in groups A （11.45 ±10.33 ） and B （12.28 ± 13.81 ） （P 〈0.05）. The difference between pre- and post-treatment scores on TCM syndrome was more significant in group C （12.65 ± 11.76） than in B （ 8.55 ±10. 15 ） （ P 〈 0.05 ）. Conclusion ： Bladder gasification and stasis dispersion combined with antibiotic therapy is effective for the treatment of m A chronic prostatitis, and therefore deserves wide clinical application.
