采用改良伊氏外固定器实施髋关节重建术治疗青年高位脱位型DDH31例报告

Modified Ilizarov hip revascularization in the treatment of 31 adolescent hip development dysplasia patients with high dislocation

[目的]探讨采用改良伊氏外固定器实施髋关节重建术治疗青年高位脱位型发育性髋关节发育不良(developmental dysplasia of the Hip,DDH)患者的临床效果。[方法]回顾性总结本院2006～2010年收治的31例单侧发育性髋关节高位脱位患者,行改良伊氏髋关节重建术治疗,年龄18～28岁,平均22岁,随访2.3～6.6年,平均3.4年。[结果]术前Harris hip score平均为45.86分(31～69分),术后Harris hip score平均为85.22分(72～95分),24例患者术后疗效好,4例患者术后出现针道感染,3例出现膝关节活动受限,经再次处理后所有患者对治疗效果满意,生活完全自理。[结论]采用改良伊氏外固定器实施髋关节重建术是一种治疗青年高位脱位型DDH的有效、可靠和合理的方法,值得推广。

[ Objective ] To evaluate the effect of modified Ilizarov hip revascularization in the treatment of hip development dysplasia patients with high dislocation in adolescent. [ Method] We conducted a retrospective review of 31 patients （mean age, 22 years, range, 18 -28 years） who were treated using the modified Ilizarov hip revascularization method for unilateral develop- mental dislocation of the hip from 2006 to 2010. The patients were followed up for 2.3 -6.6 years （mean,3.4 years）. [ Re- sult] The average preoperative Harris score was 45.86 （31 -69） and the average postoperative Harris score was 85.22 （72 - 95 ）. Twenty -four patients demonstrated a good response to the treatment. Nail infection was found in other 4 cases ,limitation of activities involving the knee joint was found in 3 cases. After resolution of symtoms, all patients repoted satisfactory results and were completely independent in their activities. [ Conclusion] Modified Ilizarov hip revascularization technique is an effective, reliable, and reasonable technique in treatment of developmental dysplastic hip with high dislocation in the youth.
