腰椎后路减压内固定融合术治疗退行性腰椎管狭窄症177例疗效分析 被引量:32

摘要 目的评价腰椎后路减压内固定融合术(PLIF)治疗退行性腰椎管狭窄症(DLSS)的临床效果,并分析腰椎内固定融合术对邻近节段退变的可能影响因素。方法对行PLIF的177例DLSS患者的随访资料进行回顾性分析,随访时间24~103个月。采用JOA下腰痛治疗评价标准、ODI功能障碍指数、VAS评价表、Henderson疗效标准观察手术疗效,并通过UCLA及Pearce分级标准分析术后邻近节段退变(ASD)的危险因素。结果 (1)177例患者术前JOA评分(4.1±1.127)分,术后(12.7±1.495)分,术后平均改善指数为(8.6±0.2)分,平均改善率为(79±1)%;术前ODI指数(54.86±15.87)%,术后(19.36±8.98)%,术后平均改善指数为(35.50±1)%,平均改善率为(64.71±1)%;术前VAS评分(6.4±0.2)分,术后(2.1±0.2)分,术后平均改善指数为(5.15±0.2)分;Henderson疗效标准:177例患者中优148例(83.61%),良21例(11.86%),可5例(2.82%),差4例(2.26%),优良率达95.47%。术后较术前明显改善,差异有统计学意义(P<0.05)。(2)术后邻近节段退变ASD的发生与患者年龄、性别无显著相关,与术前是否存在邻近椎间盘退变显著相关,且融合节段越多,其术后ASD退变程度越高。结论 PLIF是治疗DLSS的有效方法,术后ASD的发生与患者年龄、性别无显著关系,与术前邻近椎间盘是否存在退变呈显著相关,即术前邻近椎间盘退变是ASD的可能危险因素。
