放射性^(125)I粒子植入治疗口腔颌面部腺源性恶性肿瘤15例

The efficacy analysis of the treatment with implanting radioactive ^(125)I particle for 15 cases gland-derived oral maxillofacial malignancies

目的探讨放射性125I粒子植入治疗口腔颌面部腺源性恶性肿瘤的疗效和并发症。方法选择15例口腔颌面部腺源性恶性肿瘤患者,在治疗计划系统(TPS)引导下植入放射性125I粒子,粒子活度0.6～0.7mCi(2.22～2.59MBq),周边匹配剂量90～120Gy。术后复查并随访,观察患者的疗效和副反应。结果 15例患者均顺利完成放射性125I粒子植入,平均每例患者植入36.53颗粒子。手术后1～6个月复查计算机体层摄影术,未出现粒子移位,局部控制良好,随访6～27个月,治疗有效率为86.7%,未发现严重放疗副反应。结论 TPS引导下放射性125I粒子植入组织间近距离治疗口腔颌面部腺源性恶性肿瘤,安全可靠、创伤小,对控制复发及颈部淋巴转移近期效果显著。

Objective To investigate the clinical effect and complications of the treatment with radioactive 1^125Ⅰ implantation for gland-derived oral maxillofacial malignancies. Methods Fifteen patients with gland-derived oral maxillofacial malignancies was included in this study, the radioactive ^125Ⅰ particle was implanted with the guiding of TPS （treatment planning system）, particle activity was 0.6 - 0.7 mCi （2.22 - 2.59 MBq）, and the matched peripheral dose was 90 - 120 Gy. After operation, evaluate the effect and adverse reactions by follow-up. Results All of the patients were completed the implantation of radioactive ^125Ⅰ particle, the average number of ^125Ⅰ particle was 36.53 for each patient. After Surgery about 1 -6 months, the CT results showed no shifting of the particle, local control was well, and the effective rate was 86.7% about 6 - 27 months later without serious radiation side effects. Conclusion The implantation of radioactive ^125Ⅰ particle by the guide with TPS was safe, less invasive for gland- derived oral maxillofacial malignancies, and has obvious effect in the controlling of relapse and cervical lymph node metastasis.
