脑电双频指数用于脑干出血患者昏迷程度评估及预后判定的价值探讨

Value of bispectral index in evaluating comatose degree and prognosis of the patients with brainstem hemorrhage

目的分析脑电双频指数(BIS)用于评估脑干出血患者的昏迷程度以及预后判定的价值。方法收集2010年1月～2012年6月本院ICU病房收治的脑干出血昏迷患者45例,在入院24h内予以BIS监测,行格拉斯哥昏迷评分(GCS评分),并观察患者在6个月以后的预后情况,分析BIS值与GCS评分及预后之间的关系。结果 BIS和GCS之间呈正相关(r=0.821,P<0.05),BIS值随着GCS评分值的增加而增加,而死亡率逐渐降低;死亡率、植物状态率以及意识恢复率均与BIS呈正相关(P<0.05),预后越差,其BIS值越低(P<0.05)。结论 BIS是脑干出血患者昏迷程度的评估以及预后判定的重要指标,具有较高的应用价值。

Objective To analyze the value of bispectral index in evaluating comatose degree and prognosis of the patients with brainstem hemorrhage.Methods 45 patients with brainstem hemorrhage in the ICU of our hospital from January 2010 to June 2012 were collected,BIS monitoring was given within 24 hours,GCS score and prognosis of patients after 6 months were observed,the relationship between the BIS values and GCS score and prognosis were analyzed.Results The BIS and GCS had positive correlation（r=0.821,P＆lt;0.05）,BIS values increased with the increase of the value of GCS score,while the death rate decreased;mortality,vegetative state rate and the consciousness recovery rate with BIS had positive correlation（P＆lt;0.05）,the worse the prognosis,the lower the BIS value（P＆lt;0.05）.Conclusion Bispectral index in patients with brain stem hemorrhage assessment of the extent of the coma and an important indicator of prognosis,and has a high application value.
