110例重症手足口病患儿临床特点及治疗研究

Research of clinical characteristics and treatment of 110 cases of children with severe hand-foot-mouth disease

目的研究重症手足口病患儿的发病特点、临床表现、实验室检验结果及治疗经验体会。方法回顾性分析2011年5月～2013年5月本院儿科病房收治的临床诊断为重症手足口病患儿110例的一般情况、临床特点及治疗情况,采用糖皮质激素联合丙种球蛋白治疗,观察疗效。结果夏、秋为手足口病高发季节,分别占42.7%和43.6%;发病年龄以6岁以下患儿为主,占92.7%;男性患儿多于女性。手足口病患儿主要临床表现为发热、惊跳、精神差,分别占94.5%、82.7%及80.9%。实验室检查显示,大部分患儿有白细胞、血糖及肌酸激酶同工酶(CK-MB)升高。治疗后110例患儿,临床治愈107例,好转3例,无死亡病例。结论重症手足口病主要表现为神经系统受累以及多器官功能影响,早期识别及早期干预是治疗的关键。

Objective To research the disease characteristics,clinical manifestation,laboratory test results and treatment experiences of children with severe hand-foot-mouth disease.Methods The general situation,clinical characteristics and treatment of 110 children clinically diagnosed with severe hand-foot-mouth disease in pediatric department of our hospital from May 2011 to May 2013 were analyzed retrospectively.All patients were given glucocorticoid combined with gamma globulin treatment,then the effects were observed.Results Summer,autumn were the high-occurrence seasons of hand-foot-mouth disease,accounted for 42.7% and 43.6%.Under 6 years old was the major age of onset,accounted for 92.7%,and boys were more than girls.Fever,startle,poor spirit were the main clinical manifestations of children with severe hand-foot-mouth disease,accounted for 94.5%,82.7% and 80.9%,respectively.Laboratory examination showed that most of the children had elevated white blood cells,blood glucose and CK-MB.After treatment,107 cases were cured,3 cases were improved,there was no death case.Conclusion Severe hand-foot-mouth disease mainly shows nervous system involvement and multiple organ function influence,early identification and early intervention is the key to treatment.
