腰椎侧位片判断椎弓根螺钉进钉深度的放射解剖学研究

Radioanatomical Study of the Entrance Depth of Pedicle Screw in Lumbar Spine on the Lateral X-ray Film.

目的 :探讨用腰椎侧位片判断椎弓根螺钉进钉深度的可行性。材料和方法 :在 31套腰椎骨标本上 (310个椎弓根 )用 1mm软钢丝在椎弓根最狭窄处缠绕 ,按人字嵴顶点进针方法打入椎弓根直钢丝 ,拍正侧位片及横断面片 ,观测直钢丝在椎弓根及椎体内的位置 ,并推算进钉深度比率。结果 :腰椎侧位片上椎弓根螺钉进钉深度比率为L1(88.1±5 ) % ,L2 (86 .2± 4.4) % ,L3(87.7± 4.4) % ,L4 (87.2± 5 .1) % ,L5(88.1± 5 .1) %。结论 :在腰椎侧位片上可判定椎弓根螺钉进钉深度 ,其最佳比率L1～L5为 85 %～ 90 %。

Objective: To study the depth of pedicle screw in lumbar spine on lateral X-ray film.Methods:The 310 pedicles(in 31 vertebrae) twined with the wire of 1.0mm in diameter were taken for radiological study.The anteroposterior,lateral and superoinferior roentgenograph were performed,the relationship between the projection of pedicle screw and that of vertebrea on lateal x-ray films was investigated.Results:The depth rate of pedicle screw was 88.1%±5%in L 1,86.2%±4.4%in L 2,87.7%±4.4%in L 3,87.2%±5.1%in L 4,88%±5.1%in L 5.Conclusion:The entrance depth of pedicle screw can be determinated on lumbar lateral X-ray film,the best depth rate was 85%～90%in L 1～L 5.
