摘要 目的 利用PET判断恶性肿瘤复发并进行再分期。方法 应用18F FDGPET于 2 3例恶性肿瘤 ,以胸腹部断层显像和标准化摄取值 (SUV)评价 7例 (A组 )无肿瘤复发和 16例 (B组 )经病理和临床证实的肿瘤复发病人。结果  6例无肿瘤复发病人PET表现与正常人相似。 1例放射性肺炎病人于放疗部位可见轻中度放射性摄取增高 ,其他部位未见异常浓聚灶。 16例肿瘤复发病人影像分为三型 :3例仅在原发肿瘤部位复发 ;8例于原发部位同时在其它部位可见肿瘤灶 ;5例原发部位未见异常但在其它部位出现放射性浓聚灶。 11个原发部位复发的肿瘤灶 ,平均SUV 4 38± 1 84,33个其它部位肿瘤灶 ,平均SUV 2 18± 0 96。 16例病人TNM分期 ,Ⅱ期 3例 ,Ⅲ期 4例 ,Ⅳ期 9例。结论 经一次18F FDGPET全身显像即可准确判定有无恶性肿瘤复发。 Purpose Using 18 F-FDG PET to detect the malignant tumor tecurrence and its restaging. Methods 18 F-FDG PET scan on the chest and abdomen was performed in 23 patients with malignant tumor history.Among them,7 cases (group A) didn't have tumor recurrence and 16 cases (group B) had recurrences that were proven by pathology and clinical.The standard uptake value (SUV) was also applied to help evaluation. Results In group A,normal radioactivity distribution was found in 6 cases and 1 case with radiation pneumonia had slight high uptake at the right upper lobe of lung.In group B,the image patterns were classified into three types:①tumor recurrences at primary sites in 3 cases; ②tumor recurrence at primary and other sites in 8 cases; ③no recurrences at primary sites but metastatic foci found in other sites in 5 cases.The mean SUV±1SD of 11 recurrent lesions at primary sites was 4 38±1 84 and 2 18±0 96 for 33 metastatic foci.Tumor restaging of 16 patients included stage Ⅱ in 3 cases,stage Ⅲ in 4 cases and stage Ⅳ in 9 cases. Conclusion 18 F-FDG PET is a valuable tool in detecting malignant tumor recurrence and its restaging. [
