椒目瓜蒌汤合苓桂术甘汤联合西药治疗饮停胸胁型结核性渗出性胸膜炎随机平行对照研究

Randomized Controlled Study on Jiaomugualou Decoction and Lingguizhugan Decoction Combined with Western Medicine in Efficacy of Tuberculous Exudative Pleurisy

[目的]观察椒目瓜蒌汤合苓桂术甘汤联合西药治疗饮停胸胁型结核性渗出性胸膜炎疗效。[方法]使用随机平行对照方法,将71例住院患者按住院先后顺序编号方法随机分为两组,对照组35例常规西药治疗,治疗组36例椒目瓜蒌汤合苓桂术甘汤(椒目12g,瓜蒌皮15g,葶苈子9g,桑白皮12g,苦杏仁10g,枳壳15g,茯苓30g,桂枝15g,白术15g,柴胡10g,甘草6g),西药治疗同对照组,连续治疗1个月为1疗程。观测临床症状、胸腔积液、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组治愈23例,显效6例,有效5例,无效2例,总有效率94.44%。对照组治愈12例,显效8例,有效7例,无效8例,总有效率77.14%。治疗组疗效优于对照组(P<0.05)。胸腔积液消失时间治疗组优于对照组(P<0.01)。[结论]椒目瓜蒌汤合苓桂术甘汤联合西药治疗饮停胸胁型结核性渗出性胸膜炎促进胸腔积液吸收,疗效满意。

[ Objective ] Observed the efficacy of Jiaomugualou Decoction and Lingguizhugan Decoction combined with western medicine in Tuberculous exudative pleurisy. [ Method ] The study used randomized controlled method.71 patients were sequentially numbered randomly divided into two groups, control group of 35 patients used conventional western medicine, the treatment group of 36 patients used Jiaomugualou Decoction and Lingguizhugan Decoction （Jiaomu12g, Gualoupi 15g, Tinglizi 9g, Sangbaipil2g, kuxingrenl0g, zhiqiao15g, Fuling30g, Guizhi15g, Baizhu15g, Chaihul0g, Ganeao6g）combined with western medicine, continuous treatment of a course of treatment. Observed clinical symptoms, pleural effusion disappeared and adverse reactions, and determined the efficacy after two courses of treatment. [ Result ] Treatment group, 23 cases were cured, 5 cases had significant effect, 3 cases had effect, 5 cases had no effect, the total effective rate was 86.11%. Control group, 12 cases were cured, 7 cases had significant effectin, 5 cases had effect, 11 cases had no effect, the total effective rate was 68.57%. The treatment group was more than the control group in the total effective rate （P〈0.05） . The treatment group was more than the control group in Pleural effusion disappeared （ P〈0.01 ） . [ Conclusion ] Jiaomugualou Decoction and Lingguizhugan Decoction combined with western medicine can promote absorption of pleural effusion in treating tuberculous exudative pleurisy, and the results were satisfactory.
