补肺活血胶囊结合西药治疗老年慢性阻塞性肺疾病稳定期睡眠障碍的疗效评价

Therapeutic evaluation of combination of “Bufei Huoxue Capsule”and western medicine for sleep disorders in geriatric chronic obstructive pulmonary disease

目的评价补肺活血胶囊结合西药治疗老年慢性阻塞性肺疾病(COPD)稳定期患者睡眠障碍的临床疗效。方法将133例存在睡眠障碍的老年COPD稳定期患者随机分为治疗组(67例)和对照组(66例);两组均予西医抗COPD常规药物治疗,治疗组同时加服补肺活血胶囊,两组疗程均为6周。利用"便携式加速度记录仪"客观量化观察患者服药前与服药6周后的日常活动与生理节律(DP-BR)、入睡延迟时间(SL)、睡眠效率(SE)、昼间活动量(AA)及最低5小时活动量(L5)的变化情况。结果①治疗6周后,共有118例患者完成临床观察,治疗组61例,对照组57例。②治疗前后组内比较,治疗组SE、SL、AA、L5差异均有统计学意义(P<0.05),对照组无明显变化(P>0.05)。DP-BR较前改善,夜间活动量明显减少,昼间活动增加。③组间治疗后比较,各指标差异均有统计学意义(P<0.05,P<0.01)。结论补肺活血胶囊能够改善老年COPD稳定期患者的睡眠障碍,DP-BR、SL、SE、AA及L5或可作为评估COPD睡眠障碍的有效手段。

Objective To evaluate the clinical efficacy of ＂Bufei Huoxue Capsule＂ and western medicine in treating sleep disorders in geriatric chronic obstructive pulmonary disease （COPD） at stable phase. Methods One hundrand and thirty-three cases with sleep disorders of COPD were randomized into treatment group （67 cases） and control group（66 cases）. The cases in both groups were treated by western medicine, and the casea in treatment group were treated by ＂Bufei Huoxue Capsule＂, with the course of 6 weeks. The changes of daily profiles and biorhythms （DP-BR）, sleep latency （SL）, sleep efficiency （SE）, awaken activity （AA） and least active 5-h （ L5 ） were detected by portable acceleration recorder before or after 6 weeks of medication. Results （1） After 6 weeks of treatment, a total of 118 cases were observed, sixty-one cases in treatment group and 57 cases in control group. （2） After 6 weeks in treatment group, the DP-BR and AA were increased, and night activity was decreased. （3） After 6 weeks in treatment group, the SL, SE and L5 were increased significantly （ P 〈 0. 05 or P 〈 0.01 ）, and different from that of the controls significantly （ P 〈 0.05 or P 〈 0. 01 ）. Conclusion ＂Bufei Huoxue Capsule＂ is effective to improve the sleep disorders in geriatric COPD at stable phrase. The DP-BR, SL, SE, AA and L5 are the effective measures for the evaluation of sleep disorders in COPD.
