16层螺旋CT肝脾体积测量对病毒性肝炎后肝硬化的严重程度评价 被引量:1

摘要 目的 评价16层螺旋CT肝脾体积的测量对病毒性肝炎后肝硬化的严重程度与肝功能Child分级相关性的价值.方法 对42例经临床资料、B超证实的肝硬化,其中肝功能Child-Pugh分级A级21例,B级11例,C级10例,行16层螺旋CT容积扫描.采用仪器自身携带的软件,分别测量肝脏左外叶、全肝及脾脏的体积,计算左外叶/全肝的百分比,脾/肝的百分比,将上述观察指标与Child-Pugh分级行相关分析.结果 A级与B、C级之间差异有统计学意义,在肝硬化疾病严重程度评价中全肝体积及肝左外叶体积越大,肝功能越好,肝硬化各组脾脏体积及脾/全肝比均明显高于正常组(P<0.05);Child A、B、C级脾脏体积分别为(425.00±110.95)(485.00±90.47)(872.00±356.81),Child A、B、C级脾脏体积有统计学差异(P<0.05),Child A、B、C级间脾脏体积/全肝体积比差异均有统计学意义(P>0.05).结论 多层螺旋CT能准确评价肝硬化严重程度,全肝体积随Child-Push分级呈逐级缩小,左外叶体积随Child-Push分级呈逐级缩小,脾脏体积及脾/肝体积比随Child分级增加而增大.
