摘要
目的利用功能磁共振研究正常人手运动相关脑功能区的激活情况,探讨精细运动与简单运动的共同及相对特异性机制。方法 15名正常志愿者左右手分别进行腕关节屈伸、握拳运动和轮指运动,实时进行功能磁共振成像(BOLD-fMRI)扫描,使用SPM2软件对功能数据进行ANO-VA分析及多重比较分析。结果 ANOVA分析结果显示,双手握拳运动较腕关节屈曲运动明显激活对侧中央后回及顶下小叶缘上回,轮指运动较握拳运动明显激活对侧运动前区。联合作用分析显示,双手不同方式运动共同激活对侧初级运动皮层区、对侧辅助运动区及同侧小脑半球激活。交互作用分析显示,对侧初级运动皮层区及顶下小叶精细运动较简单运动激活效应具有显著差异。结论功能磁共振成像可用于检测正常人手运动相关功能区,手的精细运动与简单运动涉及相同及相对特异性脑区,这为手运动功能区受累的脑肿瘤及脑梗死的治疗及康复提供重要依据和方法。
Objective To study the use of normal hand motion and activation of related functional brain areas,functional by magnetic resonance imaging and discuss the complex and simple movement of the common and relative specificity mechanism.Methods 15 normal volunteers conducted flexion and extension,fist and treamolo movement,and at the same time,the real-time fMRI scanning was done,and ANOVA analysis and multiple comparison analysis on the function of data were also performed using SPM2 software.Results The results of ANOVA(P0.05),the hands fist movement significantly activated contralateral postcentral gyrus and inferior parietal lobule su-pramarginal gyrus compared with wrist flexion movement,and the lateral premotor area was activated significantly on movement of tremolo compared with the fist movement.The conjunction effect of different ways of motion analysis shows,both hands common activated contralateral primary motor cortex,contralateral supplementary motor area and ipsilateral cerebellar hemisphere activation.Interaction analysis showed the complex motion had significant differences in contralateral primary motor cortex and inferior parietal lobule activation effect compared with the simple movement.Conclusion Functional magnetic resonance imaging can be used for the detection of function area in normal,complex and simple hand motion which involves the same and relatively specific brain areas,providing important basis and method for the treatment and rehabilitation of brain tumors and cerebral infarction with involvement of the hand motor function area.
