曲美他嗪联合阿托伐他汀对老年缺血性心肌病患者心功能和肌钙蛋白水平的影响

Influence of trimetazidin combined with atorvastatin in cardiac functionand troponin I in elderly patients with ischemic cardiomyopathy

目的探讨曲美他嗪联合阿托伐他汀对老年缺血性心肌病(ICM)患者心功能和肌钙蛋白I水平的影响。方法 68例老年ICM合并心力衰竭患者随机分为观察组与对照组,对照组予常规抗心衰加阿托伐他汀治疗,观察组在对照组基础上联合应用曲美他嗪,观察2组疗效及治疗后患者心功能及血清肌钙蛋白I(cTnI)的变化。结果治疗6月后,观察组总有效率为91.18%,明显高于对照组73.53%,差异有统计学意义(P<0.05);治疗后2组患者cTnI及左室收缩末期内径(LVESD)、左室舒张末内径(LVEDd)及左室射血分数(LVEF)等指标较治疗前均有显著改善(P<0.05),且观察组均明显优于对照组,差异有统计学意义(P<0.05)。结论曲美他嗪联合阿托伐他汀治疗老年ICM合并心力衰竭患者,可以提高疗效,降低血清cTnI,明显改善心肌重构,提高患者运动耐量。

Objective To investigate the influence of trimetazidin combined with atorvastatin in cardiac function and troponin I in elderly patients with ischemic cardiomyopathy. Methods Sixtyeight elderly patients with ischemic cardiomyopathy （ICM） with heart failure were randomly divided into observation group and control group. The control group received conventional antiheart failure and atorvastatin treatment, In addition to the treatment of the control group, the observation group received trimetazidin. Treatment effects, cardiac function and cardiac troponin I （cTnI） changes of the two groups were observed. Results After 6 months＇ treatment, the total efficacy rate in the observation group was 91.18%, markedly higher than that in the control group （73.53 % ）. The difference was of statistical significance （ P 〈 0.05 ）. The cTnI and LVEDD, LVESD, LVEF and other indicators in the two groups improved （P 〈 0.05）, but these indicators in the observation group was remarkably better than the control group （P 〈 0.05）. The difference showed statistical significance. Conclusion The treatment in elderly ICM patients with trimetazidin combined with atorvastatin can improve the therapeutic effect and reduce cTnI, and improve the cardiac function and exercise tolerance.
