老年2型糖尿病患者视网膜病变与下肢动脉病变的相关性分析

Correlation analysis of lower extremity arterial disease and retinopathy in elderly type 2 diabetic patients

目的探讨2型糖尿病患者视网膜病变与下肢动脉病变的相关性。方法分析本院内分泌科诊治的140例2型糖尿病患者的临床资料,包括年龄、病程、收缩压、空腹血糖、甘油三酯、糖化血红蛋白、总胆固醇、高密度脂蛋白胆固等。分析不同类型2型糖尿病视网膜病变患者下肢动脉病变的发生情况,并对二者的相关性进行探讨。结果无视网膜病变组中,糖尿病下肢动脉病变(PAD)发生率为11.1%;单纯型视网膜病变组中,PAD发生率为47.5%;增殖型视网膜病变组中,PAD发生率为71.4%,三者呈递增趋势,两两比较差异均有统计学意义(P<0.05)。年龄、病程、SBP及糖化血红蛋白为糖尿病视网膜病变危险因素(P<0.01),而年龄、病程、SBP、糖化血红蛋白及甘油三酯为糖尿病下肢动脉病变危险因素(P<0.01)。其中,年龄、病程、SBP及糖化血红蛋白为二者共同危险因素。结论随着2型糖尿病患者视网膜病变加重,下肢动脉病变发生率逐渐增高,年龄、病程、SBP及糖化血红蛋白为二者共同危险因素。

Objective To investigate the correlation between lower extremity arterial disease and retinopathy in elderly type 2 diabetic patients. Methods One hundred and forty patients with type 2 diabetes in our hospital were chosen, and their the clinical data were analyzed, including age, disease duration, systolic blood pressure, fasting glucose, triglycerides, glycosylated hemoglobin, total cholesterol, high density lipoprotein cholesterol. The incidence of lower extremity arterial disease in patients with different types of retinopathy was analyzed, and the risk factors were investigated. Their correlation was analyzed. Results Among NDR, PAD, SDR patients, the incidence of PAD was 11.1%, 47.5 % and 71.4 %, showing an increasing trend, pairwise comparisons being statistically significant （P 〈 0.05）. Age, duration, SBP and glycosylated hemoglobin were risk factors of diabetic retinopathy （P 〈 0.01 ）. Age, duration, SBP, glycated hemoglobin and triglycerides were risk factors of diabetic lower extremity arterial disease （P 〈 0.01 ）. Among them, age, duration, SBP and glycosylated hemoglobin were both common risk factors. Conclusion As retinopathy of patients with type 2 diabetes increased, the incidence of lower extremity arterial disease gradually increased. Age, duration, SBP and glycosylated hemoglobin are common risk factors of both.
