重庆市南岸区近7年新生儿CH筛查结果分析

Analysis on Result of Neonatal Screening for CH in Nan'an District of Chongqing over the Last Seven Years

目的了解新生儿先天性甲状腺功能减低症(Congenital Hypothyroidism,CH)的发病情况,探讨筛查存在的问题,提出干预措施,以提高新生儿疾病筛查率。方法收集2006年1月-2012年9月重庆市南岸区出生的新生儿CH筛查资料,筛查采用时间分辨荧光免疫法检测促甲状腺激素。结果全区活产数共28 986人,共筛查27 866例,筛查率96.14%;CH初筛阳性198例,初筛阳性率7.09‰;确诊为CH17例,患病率为0.61‰。公立医院与私立医院的总体筛查率比较,差异无统计学意义;三级医院与二级及以下医院的总体筛查率比较,差异无统计学意义。结论该区CH发病率高于全国水平。应做好新生儿疾病的筛查工作,及时进行干预,避免患儿重要脏器出现不可逆性损害,保障儿童正常的体格发育和智能发育。

Objective To have knowledge about the incidence of congenital hypothyroidism（CH）in neonates,probe into problems existing in the screening and put forward intervention measures so as to improve the neonatal screening rate.Method Data of screening for CH in neonates born in Nan＇an District of Chongqing from January,2006to September,2012was collected,and thyroid stimulating hormone was detected using time resolved fluoroimmunoassay in the screening.Results Number of live-birth in the whole district was 28 986and 27 866cases were screened with a screening rate of 96.14%;there were 198positive cases in the preliminary screening for CH with a positive rate of 7.09‰;there were 17cases who were diagnosed with CH with a morbidity rate of 0.61‰.There was no statistically significant difference between overall screening rate in public hospitals and private hospitals;and there was no statistically significant difference between overall screening rate in tertiary level hospitals and secondary level or lower level hospitals.Conclusion Incidence rate of CH in this district is higher than the national level.Screening for diseases in neonates shall be conducted and intervention shall be made in time in an attempt to avoid irreversible damages in vital organs of the affected neonates and insure normal physical and intelligence development of children.
