颈椎病的CT影像学诊断价值探讨

Discussion on Diagnostic Value of CT Imaging in Cervical Spondylosis

目的探讨颈椎病CT影像学特点及其诊断价值。方法回顾性分析206例颈椎病患者的临床病历资料及CT征像。结果神经根型82例,CT显示钩突纵径增大伴后缘骨质增生,小关节突前后增生及椎间盘侧突。脊髓型62例,CT显示椎体后缘骨质增生、后纵韧带骨化、椎管狭窄、黄韧带肥厚及椎间关节内缘增生,椎间盘膨隆式突出。椎动脉型35例,CT显示钩突横径增大伴侧缘增生,横突孔狭窄或直接压迫椎动脉。混合型27例,CT表现可见以上各型,无明显特异表现形式。结论 CT检查可充分显示颈椎椎体及椎间隙解剖结构、骨质增生的范围和程度、椎间盘的情况以及韧带的病变,为临床分型提供可靠、准确的诊断依据。

Objective To probe into the features of CT imaging in cervical spondylosis and its diagnostic values.Method Clinical data and CT images of 206cases of patients with cervical spondylosis were analyzed retrospectively.Results There were 82cases of nerve root type,which in the CT were shown as increased vertical diameter of uncinate process with hyperosteogeny at the rear edge,hyperplasia of the front and rear parts of small joint axon and scoliosis of intervertebral disc.There were 62cases of spinal cord type,which in the CT were shown as hyperplasia at rear edge of the vertebral body,ossification of rear longitudinal ligament,narrow spinal canal,hypertrophy of subflavous ligament,hyperplasia at inner edge of intervertebral joint and bulgetype protrusion of the intervertebral disc.There were 35cases of arteriae vertebralis type,which in the CT were shown as increased horizontal diameter of uncinate process with hyperplasia at the lateral edge,narrow transverse foramen or direct compression of the arteriae vertebralis.There were 27cases of mixed type,of which the CT findings were the same as the above-mentioned types,no obvious characteristic manifestations were found.Conclusion CT examination can fully manifest the anatomical structure of cervical vertebral body and intervertenral space,range and degree of the hyperosteogeny,situation of the intervertebral disc and pathologic changes of the ligament,which provide reliable and accurate diagnostic basis for clinical classification.
