摘要
目的:对结核分枝杆菌复合群菌种鉴定的传统方法与PCR法进行对比研究,为临床应用提供科学依据。方法:将临床确诊结核病患者的317份痰标本做罗氏培养,分别用传统培养法和PCR法进行鉴别菌种,随机抽出一部分进行测序(金标准),用金标进一步鉴定两种结果的可靠性。结果:传统培养法敏感度、特异度、符合率分别为55.68%、58.33%、54.00%,而PCR法检测的敏感度、特异度、符合率分别为98.86%、50.00%、93.00%,明显优于传统培养法,两者检测的总符合率为53.00%,检测结果间差异有显著性(x^2=918.64,P=0.00)。结论:传统培养法检测分支杆菌菌种的结果不符合临床检验要求,且所需时间长,而PCR法方便、快速,可以较准确地确定分支杆菌菌种。
Objective: By two kinds of examinations and appraisal methods of Mycobacterium type analysis of Kashikar region tuberculosis. Evaluate methods of identification strains of mycobacterium. Providing the scientific basis for its clinical application, Methods : Clinical diagnosis of patients with tuberculosis through sputum samples in roche training. Extraction of mycobacteria strains with traditional culture medium respectively; again appraisal method and the multiple PCR to identify strains. Random drew some of the specimen, sequenced them with the gold standard for identification of the results for further reliability. Results:The traditional media culture method sensitivity,speci- ficity,fiberoptic duetoseopy are: 55.68%,55.33% .54.00%. while the multiple PCR sensitivity,specificity,fiberoptic ductoseopy are: 98.86%,50.00% .93.00% .With both total detection was at 53.00%. The test results show there is a significant difference (X2= 918.64, P=0.00). Conclusion: The results of the improved trditional culture method that is widely used but does not conform to the clinical inspection requirements and the time required is much longer while the multiple PCR method is convenient, fast and can accurate for strains of mycobacteria.
