超声生物显微镜对睫状体平坦部微小异物诊断的应用价值

The value of ultrasound biomicroscopy ( UBM) in diagnosis of micro-sized foreign body in pars plana

目的探讨超声生物显微镜(UBM)检查对于眼前节微小异物的诊断价值。方法对12例(12只眼)机械性眼外伤患者经眼科常规超声检查未探及明确异物,而依据患者病史及临床检查高度怀疑存在球内异物可能的患者应用UBM检查。主要指标:眼前节及睫状体平坦部超声图像。结果 12例患者均被发现存在微小异物,其中房角异物1眼,晶状体异物1只眼,睫状体平坦部异物10只眼,8只眼眼眶CT检查同时提示眼部异物的存在。结论 UBM检查是诊断睫状体平坦部异物,尤其是微小异物的重要手段,是传统检查手段的有效补充。

Objective To evaluate the application of ultrasound biomicroscopy（UBM） in diagnosis of micro sized foreign body in anterior segment.Methods In this systemic retrospective case study,we analyzed medical histories of 12 patients（4 left eyes and 8 right eyes） who suffered from mechanical eye injuries and were treated in Shanghai First People＇s Hospital between 2004 and 2011.No foreign body could be found with initial 10MHz ophthalmic ultrasound probe examination,although clinical history and routine ophthalmic examination highly suggested the present of intraocular foreign body.These eyes were therefore examined with UBM focused on anterior segment and pars plana.Results Micro-sized foreign body was found in anterior segment in all 12 eyes.Locations included anterior angle（1 eye）,lens（1 eye）,and pars plana（10 eyes）.In 8 eyes,the presence of ocular foreign body was further suggested by orbital CT scan results.Conclusion Ultrasound biomicroscopy is an essential examination for possible foreign bodies,particularly of very small sizes,in anterior segment and pars plana.It is a necessary complement to routine ophthalmic examinations.
