摘要 背景 右旋美托咪定(dexmedetomidine,DEX)作为一种高选择性的α2肾上腺素能受体激动剂,具有剂量依赖性的镇静、催眠和镇痛作用,同时还具有交感神经抑制、稳定血流动力学、抑制应激反应以及无呼吸抑制等药理学性质,因此被越来越多地用于清醒开颅手术围术期. 目的 综述DEX在清醒开颅手术应用的相关进展. 内容 阐述清醒开颅手术的麻醉挑战、DEX的药理学特点及其在清醒开颅术的具体应用. 趋向 DEX应用于清醒开颅手术具有独特的优势,其应用前景广阔. Background Dexmedetomidine (DEX) is a highly selective α2-adrenoreceptor agonist with dose-dependent sedative and analgesic effects.Its sympatholytic and antinociceptive properties provide hemodynamic stability without respiratory depression.These unique properties of DEX make it a potentially advantageous sedative agent for awake craniotomy.Objective To summarize the advances of DEX use in awake craniotomy.Content We elaborate the anesthetic challenges in awake craniotomy,pharmacological properties of DEX and its application in awake craniotomy.Trend DEX has been shown to offers distinct advantages for awake craniotomy,and may be used widely in awake craniotomy in the future.
