45例喉罩通气全身麻醉的临床应用体会

Clinical Experience of General Anesthesia in 45 Cases of Laryngeal Mask Airway

目的:研究喉罩通气全身麻醉在临床手术中的应用效果,为临床全身麻醉手术采取合理麻醉方法提供参考依据。方法:将90例择期手术的患者随机分为气管插管组(A组)45例和喉罩组(B组)45例,比较并记录两组插入气管导管或喉罩时(S1)、术中、苏醒时(S2)的心率(HR)和血压(BP)的变化情况;比较两种麻醉方法的通气效果的差异和术后并发症情况。结果:两组术中平均心率和血压变化不大,差异无统计学意义(P>0.05);与气管插管组(A组)相比,B组S1和S2期平均心率和血压波动明显降低,差异有统计学意义(P<0.05);血氧饱和度及呼气末二氧化碳分压结果表明两组通气效果无差异;术后并发症情况表明:B组躁动、咽痛、喉痉挛、声音嘶哑等不良反应的发生率明显低于A组(P<0.05)。结论:喉罩通气全身麻醉对血液循环和气道的刺激作用较小,术后并发症较少,病人苏醒较快,麻醉效果可靠,临床应用效果较好。

Objective： To study laryngeal mask airway in general anesthesia effect in clinical surgery,and take reasonable anesthetic methods provide a reference for clinical systemic anesthesia.Methods：90 patients undergoing elective surgery patients were randomly divided into the endotracheal intubation（Group A） 45 cases of 45 cases and in the LMA group（Group B）,and to record the two groups to insert the endotracheal tube or laryngeal mask（S1） wake（S2） heart rate（HR） and blood pressure（BP） changes;compare two anesthetic methods to block the analgesic effect of differences in postoperative complications in surgery.Results：The two groups in heart rate when S1 and S2 obtained significantly higher than Group B（P0.05） in Group A,blood pressure of the two groups in the S1 and S2 obtained in Group A was significantly higher than Group B（P0.05）;analgesic block the effective rate of 97.7% in group A,group B was 100%,showed no significant difference（P 0.05）;postoperative complications： Group B agitation sore throat,laryngospasm,hoarseness,the incidence of adverse reactions was significantly lower in Group A（P0.05）.Conclusion：The laryngeal mask airway in general anesthesia stimulate blood circulation and airway is less important,less postoperative complications,the anesthetic effect is reliable,good clinical effect.
