心脏大血管损伤的急救和晚期处理

Management of acute and delayed complication or sequela for jnjury of heart and great vessels.

目的 探讨心脏大血管损伤的急诊抢救和晚期并发症、后遗症的处理。方法 收集我院1978年10日～1998年10月20年间治疗的30例心脏大血管损伤。穿透性伤27例.其中刀刺伤右心室16例,左心室4例,右心房4例,肺动脉干1例,上腔静脉1例;三棱针刺伤左心室1例,闭合性伤3例。穿透性伤亚临床型5例,局麻探查伤口确定诊断后剖胸手术;心包压塞型6例和休克型15例直接剖胸手术;心肌挫伤行剖胸探查1例;缩窄性心包炎行部分,心包切除术1例。结果 全组存活25例,死亡5例,死亡率16.7％。结论 心脏大血管损伤早期急救侧重于迅速急症手术,晚期要加强并发症和后遗症的处理。

Objective To report the management of acute and delayed complication or sequela for injury to the heart and great vessels.Methods From 1978 to 1998,30 patients with the heart and great vessels were managed surgically . Penetrating wound were found in 27 cases, blunt injuries in 3 cases. Twenty eigilt patients were treated operativelv in no time on admission. Partial pericadiogram was performed for constrictive percaditis (1 case) . Results In this group there were 25 cured and 5 death, Mortality was 16.7% . Conclusion Fast first aid treatment surgically are important factors e-mergency treatment of cardiovascular injuries. The management of delayed complication and sequela was important in the late time.
