256层螺旋计算机断层扫描对左心耳与左心房功能相关性的初步探讨 被引量:5

Preliminary Study on Functional Correlation Between Left Atrial Appendage and Left Atrium Using 256-Slice Spiral Computed Tomography

摘要 目的:利用256层螺旋计算机断层扫描(cT)冠状动脉(冠脉)成像多时相三维容积重建,初步探讨左心耳与左心房功能的相关性。方法:连续选取我院行常规冠脉CT血管造影(CTA)检查结果阴性患者40例,将原始扫描数据重建成10个时相(5%~95%,间隔10%),分析左心房、左心耳在不同心动周期中容积的变化及左心房、左心耳功能参数之间的关系。结果:在心动周期75%时相时左心耳容积(6.47±2.25)ml,是左心房容积(68.91±15.62)ml的10.65%,二者在心动周期中同步变化。左心房排血量、左心房排空分数、左心耳排血量及左心耳排空分数平均值分别为(45.51±9.66)ml、(51.03±5.10)%、(5.55±1.94)m1、(63.91±7.75)%。左心耳排血量与左心房排血量呈正相关(r=0.644);左心耳排空分数与左心房排空分数呈正相关(r=0.499)。结论:左心耳与左心房功能密切相关;256层螺旋CT实时三维容积成像可以对左心耳及左心房的容积和功能进行客观评价,为临床提供重要参考。 Objective: To preliminarily explore the functional correlation between left atrial appendage (LAA) and left atrium using 256-slice spiral computed tomography coronary angiography (CTA) with multi-phase three-dimensional volume rendering. Methods: A total of 40 patients who received the conventional CTA by 256-slice spiral CT (without cardiovascular disease) were prospectively studied. 10 data sets of transversal image were reconstructed for every 10% (5%-95%) of the R-R interval. All images were analyzed for volume changes of LAA and left atrium in a cardiac cycle, and the functional correlation indexes between LAA and left atrium were calculated as well. Results: The volume of LAA (6.47 ± 2.25)ml was 10.65% of the volume of left atrium (68.91± 15.62)ml, and both LAA and left atrium volume changes of were synchronous in a cardiac cycle. The left atrium ejection volume (LAEV), left atrium ejection fraction (LAEF) and LAA ejection volume (LAAEV), LAA ejection fraction (LAAEF) were (45.51 ±9.66)ml, (51.03 ± 5.10)%, and (5.55± 1.94)ml, (63.91±7.75)% respectively. The LAAEV was positively related to LAEV (r=0.644), and LAAEF was positively related to LAEF (r=0.499). Conclusion: There is close functional correlation between LAA and left atrium. 256-slice spiral CTA with multi-phase three-dimensional volume rendering may impersonally assess the functions of LAA and left atrium, therefore provide the important reference in clinical uractice.
