中药熏蒸疗法治疗腰椎间盘突出症84例临床疗效研究

Study of the Clinical Effect of Traditional Chinese Medicine Fumigation on 84 Cases of Lumbar Disc Herniation Patients

目的:探讨中药熏蒸疗法辅助常规疗法治疗腰椎间盘突出症的临床疗效。方法:将2012年1月至6月期间收治的84例腰椎间盘突出症患者随机分为对照组和熏蒸组,对照组采用腰椎牵引、药物及推拿联合治疗,熏蒸组在对照组治疗基础上进行中药熏蒸辅助治疗,治疗结束后观察2组患者的临床疗效、主要症状及体征评分、随访评分及改善率。结果:2组患者经治疗后,熏蒸组总有效率为91%,与对照组总有效率77%相比较,差异有统计学意义(P<0.05)。熏蒸组腰痛、步态、直腿抬高试验、感觉障碍、运动障碍及日常活动影响评分分别为21.4±5.3、17.6±2.5、20.4±4.1、19.4±4.1、18.9±3.5、17.0±3.6,与对照组评分为13.2±4.8、9.2±3.9、11.3±4.5、9.8±2.2、10.1±2.3、9.9±1.3相比较,各项指标毕竟差异有统计学意义(P<0.05)。熏蒸组患者3个月随访评分为21.2±5.4,改善率为78%,6个月随访评分为21.8±6.4,改善率为81%;1年随访评分为22.3±5.6,改善率为83%;与对照组患者3个月随访评分为14.9±4.5,改善率为51%;6个月随访评分为15.3±4.8,改善率为56%;1年随访评分为15.1±4.0,改善率为55%相比较,各阶段随访评分及改善率比较差异具有统计学意义(P<0.05)。结论:中药熏蒸作为一种辅助疗法,对腰椎间盘突出症具有显著辅助疗效,值得在临床进行推广。

To discuss the clinical effect of traditional Chinese medicine fumigation （TCMF） as a complementary therapy for regular treatment on lumbar disc herniation（LDH） patients and to provide clinical research basis for choosing appropriate therapeutic methods. Methods. 84 cases of LDH patients who were admitted into our hospital from Jan. 2012 to Jun. 2012 were divided into control group and fumigation group randomly, the control group was treated with lumbar traction and combination therapy of drugs and massage, while fumigation group was treated with TCMF in addition to the methods for control group. The total effective rate, grade of cardinal symptom, sign and follow-up visit, recovery rate were selected as the indicators in this study after treatment. Results： The total effective rate of fumigation group was 91% , compared with 77% of control group, has a significant difference（P 〈 0. 05 ） , and the grades of lumbago, gait, test of straight leg, sensory disorder, dyskinesia, daily activities affection were 21.4 ± 5.3, 17.6 ± 2. 5, 20.4± 4. 1, 19. 4±4. 1, 18. 9 ±3.5, 17.0 ±3.6 respectively, compared with 13.2±4.8, 9.2 ±3.9, 11.3 ±4. 5, 9.8 ±2. 2, 10. 1± 2.3, 9.9 ±1.3 of control group, has signifieantly increased（ P 〈 0. 05 ）, while the grades of follow-up rate, recovery rate of 3^th, 6^th and 12^th month of fumigation group were 21.2 ±5.4, 21.8 ±6.4, 22.3 ±5.6 and 78%, 81%, 83% respectively, compared with 14.9 ±4. 5, 15.3 ±4. 8, 15.1±4.0 and 51% , 56% , 55% of control group, has significantly increased（P 〈 0. 05）. Conclusion： As a complementary method for LDH treatment, TCMF has a significant effect on LDH, and has a high promoting value in clinical therapy.
