肝肿瘤患者围手术期血小板和冷沉淀的联合输注

The combined transfusion of platelet and cryoprecipitate in liver cancer during perioperative period

目的:探讨肝肿瘤患者围手术期间伴有出凝血功能障碍时科学合理进行血制品输注的方法。方法:120例分为血小板和冷沉淀联合输注组51例(联合组)和单独使用血小板组69例(对照组),输注前后进行血小板计数(PLT)、凝血酶原时间(PT)、活化部分活化凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(Fib)、DD二聚体(D-Dimer)的检测并观察临床疗效。结果:血小板和冷沉淀联合输注组患者PT、TT明显缩短;Fib(纤维蛋白原)明显增加;PLT明显增加,D-Dimer显著降低,而单独输注血小板组除了血小板数量增加外,其余指标改善不明显。联合输注组的临床症状改善情况更明显。结论:血小板和冷沉淀的联合输注能有效改善肝肿瘤患者围术期的凝血功能障碍,保障患者围术期的治疗安全。

Objective：To analyze reasonable methods about blood product transfusion in liver cancer patients with coagulation disorders. Method：120 cases were divided in group 1 with platelet transfusion （69 cases） and group 2 with Platelet and cryoprecipitate transfusion （51 cases）.Platelet counting（PLT）,Prothrombin Time （PT）,activated partial thromboplastin time （APTT）,Thrombin Time （TT）,fibrinogen （Fib）,DD Dimer （D-Dimer）were detected before and after transfusion.The clinical effects were observed at the same time. Result：PT、TT were significantly shortened ,Fib and PLT were significantly increased and D-Dimer was decreased in Group 2 while only PLT were significantly increased in Group 1.The clinical effect were improved better in group 2 compared with group 1. Conclusion：The combined transfusion of platelet and cryoprecipitate could effectively mend coagulation disorders in liver cancer cases and help to ensure the therapic safety during perioperative period.
