Clinical Analysis of Tolosa-Hunt Syndrome

目的加深临床医师对痛性眼肌麻痹即Tolosa-Hunt综合征(THS)诊断及治疗的认识。方法回顾分析13例THS的临床特点及治疗方案。结果 13例THS患者以动眼神经损害为主,多伴有头痛,激素治疗后头痛症状在48h内迅速缓解,眼肌麻痹症状恢复较慢,12例使用激素者在5～64d左右缓解,与传统的方法比差异具有统计学意义(P<0.05)。结论 THS为排除性诊断,神经影像学、活检及随访对该疾病的诊断非常重要。该病对皮质类固醇高度敏感的特点,但不应过分强调激素治疗显效的诊断价值。

Objective To deepen the cognition of clinicians to Tolosa-Hunt syndrome （THS） . Methods The clinical data of 13 cases of THS was retrospectively analyzed.Results THS is characteristic of ophthalmopathy, complicated with II, III, IV, VI, V 1 and V 2 cranial nerve lesion, it is of syndroms of pain behind the eyeball, ocular motor disturbance and sensory disability, characteristic of recurrence and high corticosteroid sensitivity. Verus to the traditional method （P〈0.05） .Conclusion Tolosa-Hunt syndrome is a rare disease with special clinical manifestations. It is a non-specific inflammation in cavernous sinus and nearby, speculated as an allergic disorder.
