全麻诱导时手工模拟血浆靶控输注丙泊酚的特点

摘要 目的探讨全麻诱导时手工模拟血浆靶控输注丙泊酚的推注方法以及对循环的影响。方法选100例需做静脉复合全麻的病人,ASA分级Ⅰ-Ⅱ,随机分为试验组和对照组,每组各50例。入室后开放静脉通道,常规监测ECG,BPSPO2,BIS。莫菲氏滴管滴入适量长托宁和芬太尼,在测管推注肌松剂后,试验组推注丙泊酚时均采用模拟血浆靶控输注方法,即先快速且以速度递减的手法在15秒钟左右推注4~5 mL,丙泊酚余量约在3 min左右以速度递减的手法推完。对照组推注丙泊酚时剂量为1.5~2 mg/kg,以1200 mL/h的速度推注,即1 mL/3 s,全量推完时间约30~50 s,二组均待病人意识丧失或BIS降至55左右,再行气管插管术,同时观察对循环的影响和麻醉的深度。结果试验组血流动力学比对照组更加平稳,全量减少约8%,且麻醉深度可靠。结论根据血浆靶控输注理念摸索出来的这种丙泊酚诱导方法更具科学性和优越性,值得推广。
