三腔气囊导尿管在结肠造口灌洗患者中的应用研究

Balloon catheters in three cavity lavage patients underwent colostomy care application

目的探讨三腔气囊导尿管应用于结肠造口灌洗的效果,以寻找最佳灌洗途径。方法选取我科2010年1月-2011年10月收治的结肠造口患者80例,随机均分为观察组与对照组。观察组40例采用三腔气囊导尿管灌洗,对照组40例采用＂一次性灌肠冲洗器＂（带普通肛管）灌洗,两组患者的灌洗量均为100~1000 mL。根据液体返流量的多少比较两种灌洗方法的效果及两组患者灌肠成功率、并发症（出血、穿孔）、瘘口周围皮肤、床单污染发生、患者舒适度和满意度情况。结果三腔气囊导尿管灌洗效果明显优于普通肛管,灌洗液返流量明显低于普通肛管,差异有统计学意义（P〈0.01）。观察组患者并发症、瘘口周围皮肤情况、床单污染发生率显著低于对照组（P〈0.05）,差异具有统计学意义。结论三腔气囊导尿管用于结肠造口灌洗不仅效果好,而且弥补了普通肛管灌洗返流量较多的缺陷,是一种理想的结肠造口灌洗方法。

[ Objective ] Discuss three cavity balloon catheter applied to the effect of lavage underwent colostomy, hnchng the best irrigation way. [ Methods ] Choose our department between January 2010 and October 2010 patients underwent colostomy in hospital, 80 cases were randomLy divided into two groups, observation group and control group 40 cases in each group.Group USES 3 cavity balloon catheter tube filling washing, control group adopts the ＂one-time enema syringe＂ lavage （with normal anal canal）, two groups of patients with the irrigation quantity for 100-1000 mL.How much liquid return flow were compared the effect of two irrigation method; Observed two groups of patients with enema success rate, complications （bleeding, perforation）, fistula surrounding skin condition, sheet pollution situation is happening, patient comfort and satisfaction. [ Results ] Three cavity balloon catheter tube filling washing effect is superior to common anal tube, tubing lavage fluid return flow significantly lower than normal, the difference was statistically significant （P〈0.01）. Observation group of patients with complications, fistula surrounding skin conditions, sheet contamination incidence was significantly lower than the control group, P〈0.05, the difference is statistically significant. [ Results ] Three chamber balloon catheter for irrigation not only underwent colostomy effect is good, and make up for the defects of ordinary anal canal irrigation return flow more, is an ideal irrigation methods underwent colostomy.
