自我管理行为与难控性透析间期体重增加值的相关性分析

The relationship between self-management behaviors and weight gains in patients with hemodialysis

目的探讨维持性血液透析患者自我管理行为与透析间期体重增加值的相关性。方法对162例血液透析患者在常规血液透析的同时进行自我管理行为护理,在干预前、干预后12个月进行自我管理水平问卷测评。结果自我管理水平与透析间期体重增加值均为负相关,干预前r=-0.63,干预后r=-0.11。自我管理水平干预前后的差异有统计学意义(t=11.49,P<0.01)。透析间期体重增加值与自我管理水平中相关性最强的是液体离子的限制,干预前r=-0.29,干预后r=-0.45;其次是饮食量、一般状态和社会心理行为、躯体社会心理活动的管理。干预前后透析间期体重增加值的差异有统计学意义(t=9.97,P<0.01)。结论通过护理干预能提高维持性血透患者的自我管理行为,有利于对透析间期体重增加值的控制。

Objective To determine the relationship between weight gains and self-management behaviors in hemodialysis patients. Methods Totally 162 hemodialysis patients received self-management interventions for 12 months. The patients＇ self-management behaviors and weight gains were investigated before and after intervention. Results The patients＇ weight gains were negatively correlated with the self-management behaviors before（r=-0.63） and after intervention（r=-0.11 ）,especially with the factors of liquid ion limitation, diet, general status, socio-psychological behaviors and physical activity. After intervention, the self-management behaviors were significantly improved（P〈0.01） and the weight gains were reduced（P〈0.01）. Conclusion Nursing interventions can improve the hemodialysis patients＇ self-management behaviors and better control the weight gains during hemodialysis.
