经翼点入路显微手术治疗破裂的急性期前交通动脉瘤

Microsurgical treatment of ruptured anterior communicating aneurysms through pterional approach at acute stage

目的探讨经翼点入路显微外科手术治疗破裂的急性期前交通动脉瘤的手术方法和疗效。方法回顾性分析我院神经外科2006-10—2011-03经翼点入路显微手术治疗22例破裂的急性期前交通动脉瘤患者的临床资料,总结手术技巧和疗效。结果本组动脉瘤均为中小型动脉瘤,术中都成功夹闭。术后随访6～12月,术后GOS分级Ⅴ级12例,Ⅳ级7例,Ⅲ级2例,Ⅰ级1例。结论对破裂的前交通动脉瘤患者急性期显微手术夹闭动脉瘤,效果满意。

Objective To investigate the methods and efficacy of microscopic surgical treatment in patients with ruptured anterior communicating artery（AcoA） aneurysms at acute stage via the pterional approach.Methods Twenty-two patients with ruptured AcoA aneurysms,admitted to our hospital from October 2006 to March 2011 and underwent microsurgery through pterional approach at acute stage,were retrospective analyzed,their surgical techniques and efficacy were concluded.Results The aneurysms in 22 patients were all middle or small size;all the aneurysms were successfully clipped.Follow-up was performed for 6~12 months.The postoperative Glasgow outcome（GOS） were grade Ⅴ in 12 cases,grade Ⅳ in 7 cases,grade Ⅲ in 2 cases,gradeⅠ in 1 case.Conclusion For patients with ruptured AcoA aneurysms,favorable outcome should be achieved through microsurgical management at acute stage.
