极低出生体重儿经颞浅静脉置入PICC导管的可行性研究

Study on the feasibility of PICC insertion via superficial temporal vein in very low birth weight infants

目的:探讨极低出生体重儿经颞浅静脉留置PICC导管的效果。方法:将79例极低出生体重儿随机分为两组,观察组39例经颞浅静脉留置PICC导管,对照组40例经贵要静脉留置PICC导管,比较两组患儿导管的留置情况及并发症发生率。结果:观察组1次穿刺成功率为97.4%,对照组1次穿刺成功率为77.5%,差异有统计学意义(P<0.05);观察组静脉炎发生率7.7%,对照组静脉炎发生率27.5%,差异有统计学意义(P<0.05);两组患儿在穿刺部位出血量、导管异位率、导管使用时间、液体外渗和导管相关感染方面差异无统计学意义(P>0.05)。结论:极低出生体重儿进行PICC置管评估时颞浅静脉应作为备选静脉。

Objective： To explore the effect of PICC insertion via superficial temporal vein in very low birth weight infants. Meth- ods： Seventy- nine very low birth weight infants were randomly divided into two groups, 39 infants in observation group were treated with PICC insertion via superficial temporal vein, while 40 infants in control group were treated with PICC insertion via basilie vein ; the situations of catheter indwelling and incidence rates of complications in the two groups were compared. Results： The successful rates of the first punc- ture in observation group and control group were 97.4% and 77.5%, respectively, there was statistically significant difference between the two groups （P 〈 0. 05 ） ; the incidence rates of phlebitis in observation group and control group were 7. 7% and 27.5%, respectively, there was statistically significant difference between the two groups （P 〈 0. 05 ） ; there was no statistically significant difference in bleeding amount of at the puncture site, the incidence rate of catheter heterotopias, the usage time of catheter, fluid extravasation and catheter- related infec- tions between the two groups （ P 〉 0. 05） . Conclusion： Superficial temporal vein should be taken as an alternative vein when indwelling PICC catheter in very low birth weight infants.
