湖北麻城市老年人农业伤害流行特征及影响因素

Prevelance and risk factors of agricultural injuries among elder people in Macheng city,Hubei province

目的了解湖北省麻城市老年人农业伤害的流行特征,分析老年人农业伤害发生影响因素。方法采用多阶段分层整群抽样方法,对湖北省麻城市772名农村≥60岁老年人进行面对面的访谈调查,运用SAS 8.2软件进行统计分析。结果老年人农业伤害发生率为13.47%,伤害原因主要是跌落伤(40.38%);多因素logistic回归分析结果显示,身心健康状况(OR=1.75,95%CI=1.08～2.84)、家中农药存放方式(OR=1.87,95%CI=1.04～3.37)、喷洒农药时穿长裤、袜、鞋(OR=0.50,95%CI=0.30～0.82)是老年人农业伤害的影响因素;喷洒农药时吸烟(OR=2.51,95%CI=1.07～5.93)是老年男性农业伤害的危险因素;而饮酒(OR=2.87,95%CI=1.16～7.09)是老年女性农业伤害发生的危险因素。结论老年人农业伤害发生率较高,需分别针对男女性老年人进行农业伤害预防的宣传教育,可控制老年人伤害的发生。

Objective To explore the prevalence and risk factors of agricultural injury among elder people in Ma- cheng city of Hubei province, and to provide preventive strategy for agricultural injury among elder people. Methods Multi-stage cluster sampling method was applied in this study. First, 3 townships was randomly selected from 19 rural towns in Macheng city. Second, 17 villages were randomly selected from 34 villages in the selected towns. Finally, face- to-face interview was conducted in the elderly（ 〉 60 years old）. SAS 8. 2 was used to analyze the associations withxz test,univariate and multivariate logistic regression. Results A total of 772 people were surveyed mad the incidence of ag- ricultural injury was 13.47% among the elderly. The leading cause of the injuries was falls（40. 38% ）. The results of analysis showed that health status （ odds ratio E OR ] = 1.75,95 % confidence interval E 95 % CI] ： 1.08 - 2. 84）, pesticide storage（ OR = 1.87,95% CI： 1.04 - 3.37 ）, and wearing protective equipments（ long pants, stock, shoes） during pesticide application（ OR = 0. 50,95% CI：O. 30- 0. 82 ）were significantly associated with the agricultural injuries. The risk factors were different between male and female elder people; smoking during pesticide application（ OR = 2. 51,95% CI： 1.07 - 5.93 ）was significantly associated with agricultural injury in the males. However, alcohol drinking （OR = 2. 87,95% CI： 1.16 -7.09）was significantly associated with agricultural injury in the females. Conclusion The findings indicated that the incidence of agricultural injury was high among elder people in rural community. It is necessary to strengthen the eld- er＇s self-protection awareness, and to improve mental and physical health level for the prevention of agricultural injuries in the elderly. factor
