头颈部CT血管造影诊断颅内外动脉狭窄的价值研究 被引量:2

摘要 目的:探讨CT血管造影(CTA)对颅内外动脉狭窄的诊断价值。方法:34例患者行CTA检查后,2周内行数字减影血管造影(DSA)检查。参照北美症状性颈动脉内膜切除试验标准(North American Symptomatic Carotid Endarterectomy Trial,NASCET)对血管狭窄进行分级。以DSA为标准,评价CTA诊断血管狭窄的准确率、敏感度、特异度。结果:CTA、DSA分别诊断血管正常418、423支,狭窄或闭塞160、155支,其中轻度狭窄分别为93、104支,中度狭窄48、33支,重度狭窄12、11支,闭塞均为7支。以DSA为标准,CTA诊断颅内外血管狭窄的准确率、敏感度、特异度、阳性及阴性预测值分别为97.1%(561/578)、96.1%(149/155)、97.4%(412/423)、93.1%(149/160)及98.6%(412/418)。CTA和DSA对血管狭窄的检出率差异无统计学意义(χ2=3.465,P=0.483)。结论:CTA对颅内、外动脉狭窄的诊断具有较高敏感度、特异度和准确率,可作为动脉狭窄筛查与诊断的常规检查。
