踝关节骨折合并下胫腓关节分离治疗38例探讨

The treatment of ankle joint fracture with separation of distal tibiofibular syndesmosis for 38 cases

目的探讨踝关节骨折合并下胫腓关节分离治疗的临床疗效。方法将我院与2011年2月至2012年2月间收治的38例踝关节骨折合并下胫腓分离的患者采用切开复位内固定的方法进行治疗,并根据Baird-Jackson踝关节评分标准比较术后疗效。结果所有患者术后随访1年,21例优,11例良,4例可,2例差,治疗优良率为84.2%,其中有2例发生断钉,出现关节不稳、骨不连等并发症,均未感染。结论踝关节骨折合并下胫腓关节分离的手术治疗方法可以显著修复踝穴与距骨的解刨关系,临床疗效显著,不良反应少,值得进行临床推广。

Objective To investigate the clinical effect of ankle joint fracture with separation of distal tibiofibular syndesmosis. Methods 38 cases of ankle joint fracture with separation of distal tibiofibular syndesmosis in our hospital from February 2011 to February 2012 were treated by open reduction and internal fixation,and evaluated the clinical effect according to the Baird-Jackson ankle joint scoring standard. Results All patients were followed-up for 1 year,21 cases excellent,11 cases good,4 cases commonly, 2 cases of treatment can be poor,the excellent and good rate was 84. 2% ,which the crew of 2 cases breakage,joint instability,nonunion and other complications were not infected. Conclusion The operation of ankle fracture plane solutions for separation of tibiofibular joint merger can repair ankle and talus significantly,have a good clinical curative effect and less adverse reaction,it is worthy for promoting.
