基于德尔菲法和层次分析法确立麻醉专科护师核心能力

Based on Delphi method and analytic hierarchy process to establish core competencies of anesthesia specialist nurses

[目的]构建国内麻醉专科护师护理实践所应具备的核心能力内容及权重。[方法]通过理论分析、文献回顾、专家访谈及行为事件访谈等方法构建麻醉专科护师核心能力初稿。运用2轮Delphi专家咨询明确麻醉专科护师核心能力的维度、条目及指标,应用层次分析法统计分析核心能力维度层及条目层的权重。[结果]经历2轮Delphi专家咨询,选入专家20名,2轮问卷有效率均为100%,专家权威系数0.84,专家协调系数检验P=0.00,表明入选专家积极程度、权威程度及协调程度较高。2轮Delphi专家咨询中专家共提出60条建议,研究小组采纳了51条。最终明确国内麻醉专科护师核心能力标准,包括3个维度14个条目45个指标。维度层中知识维度最重要,占44%;条目层中护理支持技能与评估监测知识并列最重要,占14%。[结论]通过系统研究方法明确了麻醉专科护师胜任麻醉专科护理实践所需的核心能力内容及权重。

Objective：To build content and weight of core competence that domestic anesthesia specialist nurses should have in nursing practice.Methods：Through theoretical analysis,literature review,experts interview and behavioral event interview and other methods to construct draft core competencies of anesthesia specialist nurses.Through two rounds of Delphi expert consultation to clear the dimension,items and indexes of the core competence of anesthesia specialist nurses,applying analytic hierarchy process（ahp） for statistical analysis of weight of dimension layer and item layer of core competences.Results：After 2 rounds of Delphi expert consultation,20 experts were elected,the effectiveness of two rounds of questionnaire was 100%,experts coefficient was 0.84,experts coordination coefficient test P=0.00,which showed that active degree,degree of authority and coordination degree of selected experts were higher.In 2 rounds of Delphi expert consultation,a total of 60 suggestions were put forward,and the team adopted 51 items.Finally the core competency standards of domestic anesthesia specialist nurses were defined,including three dimensions,45 indicators,14 items.Knowledge dimension was the most important in dimensional level,accounting for 44%;nursing support skills and evaluation monitoring knowledge in entry level were parallel and the most important,accounting for 14%.Conclusion：Through the systematic research methods,content and weight of core competence of anesthesia specialist nurses have been clear which are needed when they are qualified for anesthesia specialty nursing practice.
