经后路C2、C3椎弓根螺钉内固定术治疗不稳定性Hangman骨折疗效观察 被引量:1

摘要 【目的】探讨经后路C2、C3椎弓根螺钉内固定术治疗不稳定性Hangman(枢椎椎弓骨折)骨折的适应证及临床疗效。【方法】收集不稳定性Hangman骨折32例,所有患者入院后均给予颅骨牵引,颅骨牵引3~5d后行后路c。椎弓根螺钉+C3侧块螺钉植骨融合内固定术。【结果】术后神经功能恢复按ASIA分级:A级2例:1例恢复至D级、1例恢复至B级;C级16例:15例恢复至D级、1例神经症状无变化;14例D级无变化;3~6个月时复查X线片骨折均愈合,无椎动脉损伤及其他并发症。【结论】后路c2椎弓根螺钉+C3侧块螺钉植骨融合内固定术治疗不稳定Hangman骨折,安全简单,疗效可靠,可最大程度上保留上颈椎的运动功能,值得临床推广应用。
