摘要
目的探讨七氟醚麻醉对兔肝缺血,再灌注损伤(ischemia/reperfusion injury,I/RI)的影响。方法健康成年新西兰大白兔21只,按照随机数字表法分为3组(每组7例):假手术组(S组)、肝I/R组(UR组)、七氟醚麻醉组(Sev-I/R组)。结扎肝动脉建立兔肝脏I/RI模型,分别于缺血前(T1)、缺血45min(T2)和再灌注60min(T3)测定血浆丙氨酸转氨酶(aspartateaminotransferase,AST)及谷草转氨酶(alanineaminotransferase,ALT)水平,于T3测定肝组织丙二醛(malondialdehyde,MDA)含量;免疫组化染色检测c-Fos蛋白的表达情况。结果与S组比较,I/R组血浆ALT和AST活性[T2:(48±6)U/L和(48±7)U/L,T3:(66±6)U/L和(70±6)U/LI及Sev-I/R组血浆A坍和AST活性[T2:(43±7)U/L和(41±3)U/L,T3:(48±6)U/L和(50±6)U/L]、肝组织MDA含量和c-Fos蛋白表达水平均明显升高(P〈0.05或P〈0.01);Sev-I/R组所有这些实验指标均明显低于I/R组(P〈0.05或P〈0.01)。结论七氟醚对I/RI兔肝组织有保护作用,可与其降低活性氧产物有关。
Objective To investigate the effects of sevoflurane anesthesia on hepatic ischemia/reperfusion injury (I/RI) in rabbits. Methods Twenty-one healthy adult New Zealand white rabbits were randomly divided into three groups (n=7): sham operation group (S group ), liver ischemia/reperfusion group (I/R group) and sevoflurane group (Sev-I/R group ). The liver I/R injury model was established in rabbit by clamping hepatic artery for 45 min, and then reperfusing for 60 min. Blood samples were collected at before (T1) and 45 min post ischemia (T2) and 60 min post-reperfusion (T3) respectively for measurement of plasma levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT). Liver specimens were obtained at the end of the experiment for determination of malondialdehyde (MDA) content and examination of pathological changes by HE staining. The expressions of c- Fos protein were observed by immunohistochemisty. Results Compared with S group, plasma ALT and AST activity in I/R group [T2(48±6) U/L and (48±7) U/L, T3(66±6) U/L and (70±6) U/L], plasma ALT and AST activity in Sev-I/R group [T2(43±7) U/L and T3 (41±3) U/L, (48±6) U/L and (50±6) U/L ], liver tissue MDA content, and c-Fos expressions in I/R group and Sev-I/R group all increased significantly (P〈0.05 or P〈0.01 ). These indexes in Sev-I/R group were all lower than those in I/R group(P〈0.05 or P〈 0.01 ). Conclusions Sevoflurane could protect the rabbit liver from I/RI, and which may be related to the down-regulation of peroxides.
